Individualization of treatment during pregnancy in patients with bipolar disorder: main therapeutic options

Resumo: Objective: To report the outcomes observed in the treatment of patients with Bipolar Affective Disorder during pregnancy. Methodology: This is descriptive research of the integrative review type. Studies published between 2000 and 2022 were considered, obtained from scientific databases that cover relevant sources inherent to the topic, using as one of the main criteria the selection of original and current articles. In all, 27 articles that addressed the topic were analyzed. Results: The use of lithium should be done with reservations, due to the risk of neural defects in the fetus, in addition to the close monitoring of the pregnant woman's lithemia. Anticonvulsants also have a risk of neural defects, in addition to the possibility of alteration in the metabolism of contraceptives, with the risk of unwanted pregnancy. Atypical antipsychotics pose no risk to the fetus, except for risperidone. Those with a higher metabolic profile, such as quetiapine and olanzapine, have a higher risk of developing gestational diabetes. Conclusion: The mood control of patients with bipolar disorder must be competently monitored by the health professional, who must be equipped with all therapeutic options, so that he can present them to patients, thus obtaining a shared strategy with greater chances of success.

Metodología: Se trata de una investigación descriptiva del tipo revisión integradora. Se consideraron estudios publicados entre 2000 y 2022, obtenidos de bases de datos científicas que cubran fuentes relevantes inherentes al tema, utilizando como uno de los principales criterios la selección de artículos originales y actuales. En total, se analizaron 27 artículos que abordaban el tema. Resultados: El uso de litio debe hacerse con reservas, por el riesgo de defectos neurales en el feto, además del estrecho seguimiento de la litemia de la gestante. Los anticonvulsivantes también tienen riesgo de defectos neurales, además de la posibilidad de alteración en el metabolismo de los anticonceptivos, con riesgo de embarazo no deseado. Los antipsicóticos atípicos no presentan riesgo para el feto, excepto la risperidona. Los que tienen un perfil metabólico más alto, como la quetiapina y la olanzapina, tienen un mayor riesgo de desarrollar diabetes gestacional. Conclusión: O controle do humor de las pacientes con trastorno bipolar debe ser acompañado de forma competente pelo profesional de salud, que debe estar munido de todas las opciones terapéuticas, para que puedas presentar a las pacientes, obteniendo así una estrategia compartida y con mayores chances éxito.Objetivo: Relatar os desfechos observados no tratamento de pacientes com Transtorno Afetivo Bipolar durante a gestação. Metodologia: Trata-se de uma pesquisa descritiva do tipo revisão integrativa. Foram considerados os estudos publicados no período entre 2000 e 2022, obtidos de bases de dados científicas que abrangem fontes relevantes inerentes ao tema, utilizando como um dos principais critérios a seleção de artigos originais e atuais. Ao todo, foram analisados 27 artigos que abordaram o tema. Resultados: O uso de lítio deve ser feito com ressalvas, devido ao risco de defeitos neurais no feto, além do acompanhamento atento a litemia da gestante. Anticonvulsivantes também possuem risco de defeitos neurais, além da possibilidade de alteração no metabolismo de anticoncepcionais, com risco de gestação indesejada. Antipsicóticos atípicos não apresentam riscos ao feto, salvo a risperidona. Aqueles com maior perfil metabólico, como a quetiapina e olanzapina, possuem maior risco de desenvolvimento de diabetes gestacional.
