Medical approach to children victim of sexual violence

Resumo: | Introduction: Child sexual violence is highlighted among all violence committed in children, mainly because of its moral content, which involves from the family relationship to the involvement of the health team. Although not the most frequent, child sexual violence has often irreversible consequences that result in physical and psychological damage, as well as significant damage to the child's growth and development. Objectives: To describe the medical approach to children who are victims of sexual violence, in order to provide guidance to medical professionals on how to manage pediatric patients in situations of abuse. Justification: This research is relevant as it helps in the process of conducting medical care for a child victim of sexual abuse. Methodology: This is an integrative literature review carried out in the SciELO, PubMed, MEDLINE and LILACS databases, using the DeCS: “Sexual Abuse in Childhood” and “Medical Assistance”. Combined with each other by the AND operator. Studies in Portuguese, English and Spanish were included, available online in full, in the last five years (2016-2021), repeated studies between databases and that did not address the theme were excluded. Results and Discussion: In view of the above, three categories of findings emerged, in order to solve the proposed objective, which are conceptual and legal aspects of sexual crimes, medical care in cases of sexual violence against children, effectiveness of care protocols. Final considerations: We can observe the importance of the doctor during the care of a child victim of sexual abuse. Considering that the doctor is the professional who provides the first care to the child, identifying the abuse and calling the police and the guardianship council. |

Aunque no es la más frecuente, la violencia sexual infantil a menudo tiene consecuencias irreversibles que se traducen en daños físicos y psicológicos, así como daños significativos en el crecimiento y desarrollo del niño. Objetivos: Describir el abordaje médico de los niños víctimas de violencia sexual, con el fin de orientar a los profesionales médicos sobre cómo manejar a los pacientes pediátricos en situación de maltrato. Justificación: Esta investigación es relevante ya que ayuda en el proceso de realización de la atención médica a un niño víctima de abuso sexual. Metodología: Se trata de una revisión integrativa de la literatura realizada en las bases de datos SciELO, PubMed, MEDLINE y LILACS, utilizando los DeCS: “Abuso Sexual en la Infancia” y “Asistencia Médica”. Combinados entre sí por el operador AND. Se incluyeron estudios en portugués, inglés y español, disponibles en línea en su totalidad, en los últimos cinco años (2016-2021), se excluyeron estudios repetidos entre bases de datos y que no abordaran el tema. Resultados y Discusión: Ante lo expuesto, surgieron tres categorías de hallazgos, para dar solución al objetivo propuesto, que son aspectos conceptuales y jurídicos de los delitos sexuales, atención médica en casos de violencia sexual contra los niños, efectividad de los protocolos de atención. Consideraciones finales: Podemos observar la importancia del médico durante el cuidado de un niño víctima de abuso sexual. Considerando que el médico es el profesional que brinda la primera atención al niño, identificando el maltrato y llamando a la policía y al consejo de tutela.Introdução: A violência sexual infantil é destaque entre todas as violências acometidas em crianças, principalmente por conta do seu conteúdo moral, que envolve desde a relação familiar até o envolvimento da equipe de saúde. Apesar de não ser a mais frequente, a violência sexual infantil possui consequências muitas vezes irreversíveis que resultam em danos de físicos até psicológicos, além de um prejuízo significativo no crescimento e desenvolvimento da criança. Objetivos: Descrever a abordagem médica frente a criança vítima de violência sexual, a fim de promover ações de orientação aos profissionais médicos sobre como manejar pacientes pediátricos em situações de abuso. Justificativa: Esta pesquisa é relevante, pois auxilia no processo de condução do atendimento medico a criança vítima de abuso sexual. Metodologia: Trata-se de uma revisão integrativa de literatura realizada nas bases de dados SciELO, PubMed, MEDLINE e LILACS, utilizando os DeCS: “Abuso Sexual na Infância” e “Assistência Médica”. Combinados entre si pelo operador boleando AND. Foram incluidos estudos em português, inglês e espanhol, disponiveis online na integra, nos últimos cinco anos (2016-2021), foram excluidos estudos repetidos entre base de dados e que não atendiam a temática. Resultados e Discussão: Diante o exposto, emergiram três categorias de achados, a fim de sanar o objetivo proposto sendo elas aspectos conceituais e jurídicos sobre crimes sexuais, atendimento médico em casos de violência sexual contra criança, efetividade dos protocolos de atendimento. Considerações finais: Podemos observar a importância do médico durante o atendimento a criança vítima de abuso sexual.
