Public Health Challenges: penile cancer in Maranhão

Resumo: Introduction: Maranhão has the highest incidence of penile neoplasms in the world, a pathology diagnosed more frequently in men with low socio-educational level, with poor hygiene habits and from developing countries. Thus, it is essential to understand the epidemiological aspects that justify these data. Objective: to identify the epidemiological aspects, risk factors and treatment of penile neoplasms in men from Maranhão. Methods: This work is a systematic review of the literature, developed according to PRISMA, in which 09 articles were selected, provided that they answered the guiding question, elaborated through the PICO strategy, having the following inclusion criteria: articles published in English, between 2017 and 2022, in the PUBMED and BVS databases. The Boolean characters “AND” and “OR” were used, with DeCS descriptors: penile neoplasms, health policy, male urological surgical procedures, men's health. Results: the following risk factors were found: poor penile hygiene; phimosis; unprotected sex with multiple partners; HPV infection; low income; live/work in the rural area; low or no education; be married or in a stable relationship; practice zoophilia. As for treatment, lymphadenectomy, chemotherapy, radiotherapy and penectomy have been the techniques used and indicated according to the degree of involvement: penile; and lymph node, whether inguinal or pelvic. Conclusion: it is imperative that public health policies are implemented in order to deal with sociocultural issues in locations with a high incidence of penile neoplasia, thus effectively acting in favor of awareness and reversal of these indicators.

Por lo tanto, es fundamental comprender los aspectos epidemiológicos que justifican estos datos. Objetivo: identificar los aspectos epidemiológicos, factores de riesgo y tratamiento de las neoplasias de pene en hombres de Maranhão. Métodos: Este trabajo es una revisión sistemática de la literatura, desarrollada según PRISMA, en la que fueron seleccionados 09 artículos, siempre que respondieran a la pregunta guía, elaborada a través de la estrategia PICO, teniendo los siguientes criterios de inclusión: artículos publicados en inglés, entre 2017 y 2022, en las bases de datos PUBMED y BVS. Se utilizaron los caracteres booleanos “AND” y “OR”, con descriptores DeCS: neoplasias de pene, política de salud, procedimientos quirúrgicos urológicos masculinos, salud del hombre. Resultados: se encontraron los siguientes factores de riesgo: mala higiene del pene; fimosis; sexo sin protección con múltiples parejas; infección por VPH; de bajos ingresos; vivir/trabajar en el área rural; educación baja o nula; estar casado o en una relación estable; practicar la zoofilia. En cuanto al tratamiento, la linfadenectomía, la quimioterapia, la radioterapia y la penectomía han sido las técnicas utilizadas e indicadas según el grado de afectación: peneano; y ganglio linfático, ya sea inguinal o pélvico. Conclusión: es imperativo que se implementen políticas públicas de salud para enfrentar las cuestiones socioculturales en localidades con alta incidencia de neoplasia de pene, actuando de manera efectiva a favor de la concientización y reversión de estos indicadores.Introdução: O Maranhão tem a maior incidência de neoplasias penianas no mundo, patologia diagnosticada com maior frequência em homens com baixo nível socioeducacional, com precário hábito higiênico e de países em desenvolvimento. Dessa forma, é indispensável a compreensão dos aspectos epidemiológicos que justifiquem esses dados. Objetivo: identificar os aspectos epidemiológicos, fatores de risco e tratamento das neoplasias penianas em homens do Maranhão. Métodos: Este trabalho é uma revisão sistemática da literatura, desenvolvida de acordo com o PRISMA, na qual 09 artigos foram selecionados, desde que respondessem à questão norteadora, elaborada mediante a estratégia PICO, tendo os seguintes critérios de inclusão: artigos publicados na língua inglesa, entre 2017 e 2022, nas bases de dados PUBMED e BVS. Foram utilizados os caracteres booleanos “AND” e “OR”, tendo por descritores DeCS: neoplasias penianas, política de saúde, procedimentos cirúrgicos urológicos masculinos, saúde do homem. Resultados: constatou-se como fatores de risco: má higienização peniana; fimose; relações sexuais desprotegidas com múltiplos parceiros; infecção por HPV; baixa renda; morar/trabalhar na zona rural; baixa ou nenhuma escolaridade; estar casado ou em união estável; praticar zoofilia. Quanto ao tratamento, a linfadenectomia, a quimioterapia, a radioterapia e a penectomia têm sido as técnicas utilizadas e indicadas de acordo com o grau de acometimento: peniano; e linfonodal, se inguinal ou pélvico.
