Hematoma subdural espontáneo agudo: Revisión bibliográfica

Resumo: Introduction: Cases of acute subdural hematomas are considered medical emergencies that should be treated with maximum urgency and efficacy. One class of this type of brain bleeding is spontaneous, classified mainly by the absence of trauma history and with distinct etiologies. Objective: This study aimed to evaluate the clinical, epidemiological and pathophysiological aspects of spontaneous acute subdural hematoma, basing the construction of knowledge on case reports. Materials and Methods: This is an integrative literature review about the general clinical features of spontaneous acute subdural hematoma. The PICO strategy was used to develop the guiding question. Furthermore, the descriptors "Acute Subdural Hematoma"; "Spontaneous Subdural Hematoma"; "Cerebral Hemorrhage" were cross-referenced with the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO), Ebscohost, Google Scholar and Virtual Health Library (VHL) databases. Results and Discussion: A significant portion of the articles showed that the spontaneous acute subdural hematoma is caused by etiologies different from the traumatic cause, showing a difference in the precipitating factor, but keeping the typical clinical picture of subdural bleeding. Conclusion: It was possible to realize that spontaneous acute subdural hematoma is a modality of subdural bleeding, but it is precipitated by non-traumatic causes, such as arteriovenous malformations and rupture of aneurysms by hypertensive peaks. The prognosis is reserved, given its dependence on the site of involvement, as well as on the management adopted for the patient.

Una clase de este tipo de hemorragia cerebral es la espontánea, clasificada principalmente por la ausencia de antecedentes traumáticos y con distintas etiologías. Objetivo: O presente estudo teve como objetivo avaliar os aspectos clínicos, epidemiológicos e fisiopatológicos do hematoma subdural agudo espontâneo, alicerando a construção do conhecimento com base em relatos de casos. Materiales y métodos: Se trata de una revisión integradora de la literatura sobre las características clínicas generales del hematoma subdural agudo espontáneo. Se utilizó la estrategia PICO para desarrollar la pregunta guía. Además, se cruzaron los descriptores "Acute Subdural Hematoma"; "Spontaneous Subdural Hematoma"; "Cerebral Hemorrhage" con las bases de datos National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO), Ebscohost, Google Scholar y Virtual Health Library (BVS). Resultados y Discusión: Una parte significativa de los artículos demostró que el hematoma subdural agudo espontáneo es causado por etiologías diferentes de la causa traumática, evidenciando una diferencia en el factor precipitante, pero manteniendo el cuadro clínico típico de la hemorragia subdural. Conclusión: Fue posible notar que el hematoma subdural agudo espontáneo es una modalidad de hemorragia subdural, pero es precipitado por causas no traumáticas, como malformaciones arteriovenosas y ruptura de aneurismas por picos hipertensivos. El pronóstico es reservado, pues depende del sitio de compromiso, así como del manejo adoptado para el paciente.Introdução: Os casos de hematomas subdurais agudos são considerados urgências médicas que devem ser tratados com máxima eficácia. Uma classe desse tipo sangramento cerebral é a espontânea, classificada, sobretudo, pela ausência de história de trauma e com etiologias distintas. Objetivo: O presente estudo teve como objetivo avaliar os aspectos clínicos, epidemiológicos e fisiopatológicos do hematoma subdural agudo espontâneo, alicerçando a construção do conhecimento com base em relatos de casos. Materiais e Métodos: Trata-se de uma revisão integrativa de literatura acerca das características clínicas gerais sobre o hematoma subdural agudo espontâneo. Resultados e Discussão: Uma parcela significativa dos artigos demonstrou que o hematoma subdural agudo espontâneo é provocado por etiologias distintas da causa traumática, evidenciando uma diferença no fator precipitante, mas mantendo o quadro clínico típico do sangramento subdural. Conclusão: Foi possível perceber que o hematoma subdural agudo espontâneo é uma modalidade do sangramento subdural, mas é precipitado por causas não traumáticas, tais como as más formações arteriovenosas e o rompimento de aneurismas por picos hipertensivos.
