Relationships between endometriosis and the occurrence of placenta previa: a systematic review

Resumo: Objective: To analyze the association between endometriosis and the occurrence of placenta previa. Methodology: This is a systematic review carried out using the PRISMA method, in the electronic databases PubMed, Scopus and Web of Science, using the following descriptors: “endometriosis”, “endometrioma”, “chocolate cyst”, “placenta previa”, “low lying placenta” and “abnormal placenta”. Full articles, available in Portuguese, English or Spanish, were included; published between January 2016 and December 2021; who performed a comparative study between an experimental group and a control group; studies that showed the incidences of placenta previa with statistical data. Results: 11 articles made up the final sample of this study, with a higher frequency of publication in 2019 (n: 03). Most of the studies used the retrospective cohort study, with a moderate level of evidence (III). All studies compared pregnant women with or without endometriosis, all of which showed that women with endometriosis had a higher rate of placenta previa, with Odds Ratio ranging from 0.56 to 6.42 and statistically significant. Conclusion: The findings of this research suggest that, although based on a small number of studies, endometriosis may be a possible risk factor for the occurrence of placenta previa, excluding confounding factors, which were not considered for analysis. These findings can help health professionals to be more attentive to possible complications during the course of pregnancy and delivery.

Se incluyeron artículos completos, disponibles en portugués, inglés o español; publicado entre enero de 2016 y diciembre de 2021; que realizó un estudio comparativo entre un grupo experimental y un grupo de control; estudios que mostraron las incidencias de placenta previa con datos estadísticos. Resultados: 11 artículos conformaron la muestra final de este estudio, con mayor frecuencia de publicación en 2019 (n: 03). La mayoría de los estudios utilizaron el estudio de cohorte retrospectivo, con un nivel de evidencia moderado (III). Todos los estudios compararon mujeres embarazadas con o sin endometriosis, todos los cuales mostraron que las mujeres con endometriosis tenían una mayor tasa de placenta previa, con un Odds Ratio de 0,56 a 6,42 y estadísticamente significativo. Conclusión: Los hallazgos de esta investigación sugieren que, aunque con base en un pequeño número de estudios, la endometriosis puede ser un posible factor de riesgo para la ocurrencia de placenta previa, excluyendo los factores de confusión, que no fueron considerados para el análisis. Estos hallazgos pueden ayudar a los profesionales de la salud a estar más atentos a posibles complicaciones durante el transcurso del embarazo y el parto.Objetivo: Analisar a associação entre endometriose e a ocorrência de placenta prévia. Metodologia: Trata-se de uma revisão sistemática realizada por meio do método PRISMA, nas bases de dados eletrônicas PubMed, Scopus e Web of Science, por meio dos seguintes descritores: “endometriosis”, “endometrioma”, “chocolate cyst”, “placenta previa”, “low lying placenta” e “abnormal placenta”. Foram incluídos artigos completos, disponíveis em português, inglês ou espanhol; publicados entre janeiro de 2016 a dezembro de 2021; que realizaram estudo comparativo entre um grupo experimental e um grupo controle; estudos que mostraram as incidências de placenta prévia com dados estatísticos. Resultados: 11 artigos compuseram a amostra final desse estudo, com maior frequência de publicação em 2019 (n: 03). Grande parte das pesquisas utilizaram o estudo de coorte retrospectivo, apresentando nível de evidência moderado (III). Todos os estudos compararam gestantes com ou sem endometriose, onde todos demonstraram que as mulheres com endometriose tiveram uma maior taxa de placenta prévia, com Odds Ratio (Razão de probabilidade) variando entre 0,56 a 6,42 e significância estatística. Conclusão: Os achados dessa pesquisa sugerem que, embora com base em um pequeno número de estudos, a endometriose possa ser um possível fator de risco para a ocorrência de placenta prévia, excluindo os fatores de confusão, que não foi considerado para análise.
