Regenerative endodontics: integrative review

Resumo: Endodontics is the branch of dentistry that studies the pulp and its periradicular tissues, with the aim of preventing, diagnosing and treating its various diseases. With the advance in the principles of regenerative endodontics, new protocols have emerged to regenerate and preserve the pulp tissue that was once lost. Among the various techniques studied in regenerative endodontics, we highlight cell therapy, a protocol that has been little studied in the literature. The aim of this review was to investigate the feasibility of pulp regeneration techniques using dental pulp stem cells, highlighting their advantages, disadvantages, clinical efficacy, indications and limitations. An integrative literature review was conducted by searching scientific articles in the electronic databases PubMed, Medline, Scopus and Web of science using the descriptors: pulp, regeneration, transplant, indexed in the period from 2017 to 2022. After the eligibility criteria, 14 studies were selected entirely in the English language. The studies showed the feasibility of cell culture techniques, as well as their excellent clinical results, with thickening of root walls, regression of lesions and positive response to sensitivity tests. However, the techniques have high financial, operational and laboratory costs for their development, and there is a lack of clinical trials to establish a clinical protocol.

Con el avance de los principios de la endodoncia regenerativa han surgido nuevos protocolos con el fin de regenerar y conservar el tejido pulpar una vez perdido. Entre las diversas técnicas estudiadas en endodoncia regenerativa destaca la terapia celular, protocolo poco estudiado en la literatura. El objetivo de esta revisión fue investigar la factibilidad de las técnicas de regeneración pulpar utilizando células madre de pulpa dental, destacando sus ventajas, desventajas, eficacia clínica, indicaciones y limitaciones. Se realizó una revisión integrativa de la literatura a través de una búsqueda de artículos científicos en la base de datos electrónica PubMed, Medline, Scopus y Web of Science utilizando los descriptores: pulpa, regeneración, trasplante, indexados de 2017 a 2022. Después de los criterios de elegibilidad, 14 estudios fueron seleccionados íntegramente en inglés. Los estudios demostraron la viabilidad de las técnicas de cultivo celular, así como sus excelentes resultados clínicos, con engrosamiento de la pared radicular, regresión de la lesión y respuesta positiva a las pruebas de sensibilidad. Sin embargo, las técnicas tienen altos costos financieros, operativos y de laboratorio para su desarrollo, además de la falta de ensayos clínicos para la elaboración de un protocolo clínico.A endodontia é o ramo da odontologia que estuda a polpa e seus tecidos perirradiculares, com o intuito de prevenir, diagnosticar e tratar suas mais diversas doenças. Com o avanço nos princípios da endodontia regenerativa, novos protocolos surgiram a fim de regenerar e preservar o tecido pulpar outrora perdido. Dentre as diversas técnicas estudadas na endodontia regenerativa, dá-se destaque à terapia celular, protocolo há pouco estudado pela literatura. O objetivo desta revisão foi investigar a exequibilidade das técnicas de regeneração pulpar por meio de células tronco da polpa dentária, destacando suas vantagens, desvantagens, eficácia clínica, indicações e limitações. Uma revisão integrativa da literatura foi realizada por meio de uma busca de artigos científicos na base de dados eletrônica PubMed, Medline, Scopus e Web of science utilizando os descritores: pulp, regeneration, transplant, indexados no período de 2017 a 2022. Após os critérios de elegibilidade foram selecionados 14 estudos integralmente na língua inglesa. Os estudos mostraram a exequibilidade das técnicas de cultivo celular, assim como seus excelentes resultados clínicos, com espessamento das paredes radiculares, regressão de lesões e resposta positivas a testes de sensibilidade.
