Identification of acute kidney injury in an intensive care unit: parameters for clinical nursing evaluation

Resumo: The aim of this study was to identify the prevalence of acute kidney injury in Intensive Care Unit patients by isolated assessment of serum creatinine elevation and clinical application of the renal dysfunction classification Acute Kidney Injury Network. This is a prospective cohort study carried out in the Intensive Care Unit of a hospital specializing in cardiology hospital. Information was obtained from clinical and laboratory records. For the definition of acute kidney, the criteria for elevation in serum creatinine values and Acute Kidney Injury classification criteria were adopted. The final sample involved 79 patients, with a mean age of 59.8 (SD = 13.5) years. The main comorbidities were systemic arterial hypertension, congestive heart failure, diabetes mellitus and chagas disease. The prevalence of acute kidney injury was 63.3% in the evaluation of isolated serum creatinine, while, according to the Acute Kidney Injury Network criteria, 77.2% were identified. It is concluded that the Acute Kidney Injury Network criterion presents better acute kidney injury stratification than creatinine alone and can serve as an evaluation instrument in critically ill patients. As it is a method composed by usual evaluation parameters in Intensive Care Unit, it can be incorporated by the nurse in his clinical evaluation.

As it is a method composed by usual evaluation parameters in Intensive Care Unit, it can be incorporated by the nurse in his clinical evaluation.El objetivo de este estudio fue identificar la prevalencia de la lesión renal aguda  en pacientes de la Unidad de Cuidados Intensivos mediante una evaluación aislada de la elevación de la creatinina sérica y la aplicación clínica de la clasificación de disfunción renal Acute Kidney Injury Network.  Este es un estudio de cohorte prospectivo en la Unidad de Cuidados Intensivos de un hospital especializado en cardiología. La información se obtuvo de registros clínicos y de laboratorio. Para la definición de la lesión renal aguda, se adoptaron los criterios de elevación de los valores de creatinina sérica y los criterios de clasificación de Acute Kidney Injury Network.  La muestra final incluyó a 79 pacientes, con una edad media de 59,8 (DE = 13,5) años. Las principales comorbilidades fueron hipertensión arterial sistémica, insuficiencia cardíaca congestiva, diabetes mellitus y enfermedad de Chagas. La prevalencia de la lesión renal aguda fue del 63,3% en la evaluación de la creatinina sérica aislada, mientras que, según los criterios de Acute Kidney Injury Network, se identificaron el 77,2%. Se concluye que el criterio Acute Kidney Injury Network presenta una mejor estratificación de la lesión renal aguda  que  la creatinina sola y puede servir como un instrumento de evaluación en pacientes críticos. Como es un método compuesto por los parámetros habituales de evaluación de la Unidad de Cuidados Intensivos, las enfermeras pueden incorporarlo en su evaluación clínica.Este estudo teve como objetivo identificar a prevalência de lesão renal aguda em pacientes de unidade de terapia intensiva por meio da avaliação isolada da elevação da creatinina sérica e da aplicação clínica da classificação de disfunção renal Acute Kidney Injury Network. Trata-se de um estudo de coorte prospectivo realizado em unidade de terapia intensiva de um hospital especializado em cardiologia. Foram obtidas informações a partir dos registros de prontuário clínico e laboratorial. Para definição da lesão renal aguda foram adotados os critérios de elevação nos valores de creatinina sérica e critérios de classificação Acute Kidney Injury Network. A amostra final envolveu 79 pacientes, com média de idade de 59,8 (DP=13,5) anos, tendo como principais comorbidades hipertensão arterial sistêmica, insuficiência cardíaca congestiva, diabetes mellitus e doença de chagas. A prevalência de lesão renal aguda foi de 63,3% na avaliação da creatinina sérica isolada, enquanto que, pelos critérios Acute Kidney Injury Network identificou-se 77,2%. Conclui-se que o critério Acute Kidney Injury Network apresenta melhor estratificação de lesão renal aguda do que a creatinina isolada e pode servir como instrumento de avaliação em pacientes críticos.
