Effectiveness of the right of access to health of people in street situations front of the COVID-19 pandemic

Resumo: The present study sought to evaluate, based on the specialized literature, how the right of access to health by homeless people has been affected in the face of the COVID-19 pandemic. This is an integrative review research carried out through an advanced search in the SciELO, PubMed, Science Direct, BVS and LILACS databases, with selection of articles made with the following Health Sciences Descriptors (DeCS): “People on the Street”, “Right to Health”, “Access to Health Services” and “COVID-19”, and their respective terms translated into English. Among the documents and articles found, 16 were selected because they fit the purpose of the study. The barriers imposed by the COVID-19 pandemic on homeless populations are notorious, especially when the right of access to health is verified by this social group. Specifically in its most acute phase, the pandemic was and still has been marked by insufficiency in hygiene and social isolation measures for this segment. In addition, health vulnerability is observed in higher mortality rates among homeless people compared to the general population, as well as the fragmentation of mental health care aggravated by the pandemic. Strategies were taken to protect and provide health care to the homeless population. However, an insufficiency in the implementation of such strategies by states and municipalities is observed, demonstrating the need for effective health care to guarantee the rights of this population.

Se trata de una investigación de revisión integradora realizada a través de una búsqueda avanzada en las bases de datos SciELO, PubMed, Science Direct, BVS y LILACS, con selección de artículos elaborados con los siguientes Descriptores en Ciencias de la Salud (DeCS): “Gente en la Calle”, “Derecha a la Salud”, “Acceso a los Servicios de Salud” y “COVID-19”, y sus respectivos términos traducidos al inglés. Entre los documentos y artículos encontrados, se seleccionaron 16 por adecuarse al propósito del estudio. Las barreras impuestas por la pandemia del COVID-19 a las poblaciones en situación de calle son notorias, más aún cuando se verifica el derecho de acceso a la salud por parte de este grupo social. Específicamente en su fase más aguda, la pandemia estuvo y aún está marcada por la insuficiencia en las medidas de higiene y aislamiento social para este segmento. Además, la vulnerabilidad sanitaria se observa en mayores tasas de mortalidad entre las personas sin hogar en comparación con la población general, así como la fragmentación de la atención en salud mental agravada por la pandemia. Se tomaron estrategias para proteger y brindar atención en salud a la población en situación de calle. Sin embargo, se observa una insuficiencia en la implementación de tales estrategias por parte de los estados y municipios, lo que demuestra la necesidad de una atención de salud efectiva para garantizar los derechos de esta población.O presente estudo buscou avaliar, a partir da literatura especializada, como o direito de acesso à saúde pelas pessoas em situação de rua tem sido afetado frente à pandemia da COVID-19. Trata-se de uma pesquisa de revisão integrativa realizada por meio de uma busca avançada nas bases de dados SciELO, PubMed, Science Direct, BVS e LILACS, com seleção dos artigos feita com os seguintes Descritores em Ciências da Saúde (DeCS): “Pessoas em Situação de Rua”, “Direito à Saúde”, “Acesso aos Serviços de Saúde” e “COVID-19”, e seus respectivos termos traduzidos no idioma inglês. Entre os documentos e artigos encontrados, 16 foram selecionados por se adequarem ao objetivo do estudo. São notórias as barreiras impostas pela pandemia de COVID-19 às populações em situação de rua, em especial, quando se verifica o direito de acesso à saúde por esse grupo social. Especificamente em sua fase mais aguda, a pandemia foi e tem sido ainda marcada por insuficiência em medidas de higienização e isolamento social para esse segmento. Além disso, a vulnerabilidade em saúde é observada nas maiores taxas de mortalidade entre as pessoas em situação de rua comparado à população em geral, bem como a fragmentação do cuidado em saúde mental agravada pela pandemia. Estratégias foram tomadas para proteção e cuidado em saúde à população em situação de rua.
