Coordination of care and ordination of health care networks: qualitative study with higher level workers in the Family Health Strategy

Resumo: Introduction: the Unified Health System (SUS) is an organizational system to reorder health services and actions in the Brazilian territory. Thus, the need for health workers to know the system in which they are inserted is imperative, so that users feel welcomed in any scope of SUS. Objective: to know the facilitating and hindering elements in the view of five health workers, of higher level, of the Family Health Strategy (ESF), of a small municipality in the State of Rio Grande do Sul, in relation to the coordination of user care and the ordering of Health Care Networks. Methodology: this is a qualitative study, conducted in 2019, in which the research subjects answered a semi-structured qualitative interview. Results: as facilitating elements, reception were highlighted, the teleconsulting, the resoluteness of care, knowledge of the population's problems, the prepared and well-connected team, the structured care network. The hindering elements were the unorganized decentralization, the high demand, too long waiting times for assistance in medium and high complexity, structural economic difficulties and inadequate clinical management of chronic users. Conclusion: finally, is pointed that it is up to the management and the teams to reorganize and strengthen the work, in order to minimize the problems and solidify the potential, making the ESF and SUS really effective and efficient.

Por tanto, la necesidad de que los trabajadores de la salud conozcan el sistema en el que se insertan es imperativa, para que los usuarios se sientan acogidos en cualquier contexto del SUS. Objetivo: conocer los elementos facilitadores y obstaculizadores en la visión de cinco trabajadores de la salud, de nivel superior, de la Estrategia de Salud Familiar (ESF), de un pequeño municipio del Estado de Rio Grande do Sul, en relación con la coordinación de la atención al usuario y el ordenamiento de las Redes de Atención Sanitaria. Metodología: se trata de un estudio cualitativo, realizado en 2019, en el que los sujetos de investigación respondieron a una entrevista cualitativa semiestructurada. Resultados: como elementos facilitadores se destacaron la recepción, la teleconsulta, la resolución de la atención, el conocimiento de los problemas de la población, el equipo preparado y bien conectado, la red de atención estructurada. Los elementos obstaculizadores fueron la descentralización desorganizada, la alta demanda, los tiempos de espera demasiado largos para la asistencia en mediana y alta complejidad, las dificultades económicas estructurales y el manejo clínico inadecuado de los usuarios crónicos. Conclusión: finalmente, corresponde a la dirección y a los equipos reorganizar y fortalecer el trabajo, con el fin de minimizar problemas y solidificar el potencial, haciendo que la FHS y el SUS sean realmente efectivos y eficientes.Introdução: o Sistema Único de Saúde (SUS) é um sistema organizacional para reordenar os serviços e as ações em saúde no território brasileiro. Assim, é imperiosa a necessidade de que os trabalhadores em saúde conheçam o sistema no qual estão inseridos, para que os usuários se sintam acolhidos em qualquer âmbito do SUS. Objetivo: conhecer os elementos facilitadores e dificultadores na visão de cinco trabalhadores em saúde, de nível superior, da Estratégia Saúde da Família (ESF), de um município de pequeno porte do Estado do Rio Grande do Sul, em relação à coordenação do cuidado dos usuários e ordenação das Redes de Atenção à Saúde. Metodologia: trata-se de um estudo qualitativo, realizado em 2019, no qual os sujeitos da pesquisa responderam a uma entrevista qualitativa semiestruturada. Resultados: como elementos facilitadores foram destacados o acolhimento, as teleconsultorias, a resolutividade do cuidado, o conhecimento dos problemas da população, a equipe preparada e bem relacionada, a rede de atenção estruturada. Já os elementos dificultadores foram a descentralização não organizada, a alta demanda, o demasiado tempo de espera para o atendimento na média e alta complexidade, as dificuldades econômicas estruturais e a inadequada gestão clínica dos usuários crônicos.
