Analysis of the cost-effectiveness of the paleolithic diet compared to the diet supported by guidelines for weight loss in obese people: a randomized clinical trial

Resumo: | The aim of this study was to compare the cost of the paleolithic (PD) diet with that of the traditional diet supported by guidelines (DTG) in obese patients treated by the Brazilian public health system. It is a randomized clinical trial with obese patients who were divided into two groups according to the dietary prescription: DP and DTG, involving, respectively, 82 and 73 patients. They were followed up for 30 and 60 days, with two monthly 24h (R24h) reminders. A cost-effectiveness analysis of the DP and DTG diets was performed. The daily cost of each R24h was calculated with the aid of the PROCONOMIZE tool, a free application available on the IOS platform. As a measure of efficiency or outcome, the patient's weight loss variable was used, and the average cost ratio was calculated by varying the patients' weight. The cost-effectiveness calculation was carried out taking into account the cost ratio, considering the standard deviation, for efficiency. There was no difference between groups in terms of demographic, socioeconomic, nutritional status and usual expenditure on daily food before starting treatment. Regarding the average cost, the DP proved to be more economical when compared to the DTG. In addition, PD is more efficient, as it has a lower cost-effectiveness ratio, both at 30 days and 60 days, in addition to greater weight loss, when compared to DTG. PD is a viable option for treating obesity, with a similar cost to DTG. |

Se trata de un ensayo clínico aleatorizado con pacientes obesos que se dividieron en dos grupos según la prescripción dietética: DP y DTG, involucrando, respectivamente, 82 y 73 pacientes. Fueron seguidos durante 30 y 60 días, con dos recordatorios de 24 h (R24h). Se realizó un análisis de costo-efectividad de las dietas DP y DTG. El coste diario de cada R24h se calculó con la ayuda de la herramienta PROCONOMIZE, una aplicación gratuita disponible en la plataforma IOS. Como medida de eficiencia o resultado, se utilizó la variable de pérdida de peso del paciente y se calculó la relación de costo promedio variando el peso de los pacientes. El cálculo de la rentabilidad se realizó teniendo en cuenta la relación de costos, considerando la desviación estándar, para la eficiencia. No hubo diferencias entre los grupos en términos de estado demográfico, socioeconómico, nutricional y gasto habitual en alimentación diaria antes de iniciar el tratamiento. En cuanto al costo promedio, el DP resultó ser más económico en comparación con el DTG. Además, la DP es más eficiente, ya que tiene una menor relación coste-efectividad, tanto a los 30 días como a los 60 días, además de una mayor pérdida de peso, en comparación con la DTG. Eles foram acompanhados durante 30 e 60 dias, sendo realizados dois recordatórios de 24h (R24h) mensais. Foi realizada análise de custo-efetividade das dietas DP e DAD. O custo diário de cada R24h foi calculado com o auxílio da ferramenta PROCONOMIZAR, um aplicativo gratuito disponível na plataforma IOS. Como medida de eficiência ou desfecho foi utilizada a variável perda de peso dos pacientes, tendo sido calculada a razão do custo médio pela variação do peso dos pacientes. O cálculo do custo-efetividade foi realizado levando-se em conta a razão dos custos, considerando o desvio-padrão, pela eficiência. Não houve diferença entre os grupos quanto a características demográficas, socioeconômicas, estado nutricional e gasto habitual com a alimentação diária antes do início do tratamento. Em relação ao custo médio, a DP demonstrou ser mais econômica quando comparada à DAD. Além disso, a DP é mais eficiente, pois apresenta uma relação de custo-efetividade menor, tanto com 30 dias, quanto com 60 dias, além de uma maior perda de peso, quando comparada à DAD.
