Orofacial pain and its relation with hypocalcemia associated with chronic kidney disease: an overlook differential diagnosis

Resumo: Although orofacial pain is a condition of multifactorial etiopathogenesis and sometimes it is difficult to diagnose it, a possible cause for this condition is muscle changes, which in turn can originate in systemic conditions, such as hypocalcemia. The present article aims to present a clinical situation of orofacial pain due to hypocalcemia in a young female patient with chronic kidney disease. The patient attended hospital care with strong pain complaints in the face, being evaluated by the Maxillofacial Surgery team and the Neurology medical team, without the precise determination of an etiology. Despite being prescribed oral medication, the patient returned with the complaint of pain and with a new symptom of cramping in the face, in face of that, hematological tests were requested where hypocalcemia condition was found. The therapeutic approach consisted of oral calcium supplementation, which was sufficient to suppress facial complaints. It is concluded that there is a correlation between blood calcium concentration and muscle changes in the face, requiring clinical studies that better explain this relationship.

El presente artículo tiene como objetivo presentar una situación clínica de dolor orofacial por hipocalcemia en una paciente joven con enfermedad renal crónica. El paciente acudió a atención hospitalaria con fuertes quejas de dolor en el rostro, siendo evaluado por el equipo de Cirugía Maxilofacial y el equipo médico de Neurología, sin la determinación precisa de una etiología. A pesar de que se le prescribió medicación oral, el paciente regresó con la queja de dolor y con un nuevo síntoma de calambres en el rostro, ante lo cual se solicitó pruebas hematológicas donde se encontró una condición de hipocalcemia. El enfoque terapéutico consistió en suplementos de calcio por vía oral, que fue suficiente para suprimir las molestias faciales. Se concluye que existe una correlación entre la concentración de calcio en sangre y los cambios musculares en el rostro, requiriendo estudios clínicos para explicar mejor esta relación.Apesar da dor orofacial ser uma condição de etiopatogenia multifatorial e algumas vezes se ter dificuldades em diagnosticá-la, uma possível causa para esta condição são as alterações musculares, que por sua vez podem ter origem em condições sistêmicas, como por exemplo, a hipocalcemia. O presente artigo tem como objetivo apresentar uma situação clínica de dor orofacial decorrente de hipocalcemia em paciente do sexo feminino, jovem, com doença renal crônica. A paciente compareceu ao atendimento hospitalar com fortes queixas álgicas em face, sendo avaliada pela equipe de Cirurgia bucomaxilofacial e equipe médica de Neurologia, sem a determinação precisa de uma etiologia. Apesar de ser prescrita medicação oral, a paciente retornou com a queixa de dor e com uma nova sintomatologia de câimbra em face, diante disso, solicitou-se exames hematológicos onde foi constatada condição de hipocalcemia. A conduta terapêutica consistiu na suplementação oral de cálcio, sendo suficiente para suprimir as queixas faciais.
