International association of Dental Traumatology Guidelines for Dental Avulsion: The latest suggestions

Resumo: Background: Dental avulsion is characterized by the displacement of the tooth out of the alveolar cavity, causing damage to the periodontal ligament and dental pulp. Its prognosis depends on the immediate actions taken after the accident, and reimplantation is the best way to restore function and aesthetics to the patient. Objective: To compare the last three protocols of the International Association of Dental Traumatology (IADT) and evaluate the evolution of the treatment, highlighting the recommended therapy. Material and Methods: IADT guidelines published in 2007, 2012, and 2020 and articles related to dental avulsion were used,searched on the digital platforms Pubmed and Google Scholar. Classification analysis of the articles found by the Scimago Journal Rank (SJR) indicator was used. Only studies with Q1 and Q2 ratings were included. Results: Major updates in storage media with physiological osmolarity, cleaning of the avulsed tooth, antibiotic therapy, root canal treatment, root surface treatment, and clot removal from the alveolar cavity were implemented. Conclusion: Updating professionals regarding the therapeutic protocols related to avulsion is necessary for a view of the broad update regarding the recommended management strategies.

Su pronóstico depende de las acciones inmediatas que se realicen tras el accidente, siendo el reimplante la mejor forma de devolver la función y estética al paciente. Objetivo: Comparar los tres últimos protocolos de la Asociación Internacional de Traumatología Dental (IADT) y evaluar la evolución del tratamiento, destacando la terapia recomendada. Material y Métodos: Se utilizaron las guías de la IADT publicadas en 2007, 2012 y 2020 y artículos relacionados con la avulsión dental, buscados en las plataformas digitales Pubmed y Google Scholar. Se utilizó un análisis de clasificación de los artículos encontrados por el indicador Scimago Journal Rank (SJR). Solo se incluyeron estudios con calificaciones Q1 y Q2. Resultados: Se implementaron importantes actualizaciones en medios de almacenamiento con osmolaridad fisiológica, limpieza del diente avulsionado, antibioticoterapia, tratamiento de conducto, tratamiento de superficie radicular y remoción de coágulos de la cavidad alveolar. Conclusión: La actualización de los profesionales en cuanto a los protocolos terapéuticos relacionados con la avulsión es necesaria ante la amplia actualización en cuanto a las estrategias de manejo recomendadas.Introdução: A avulsão é caracterizada pelo deslocamento do dente para fora da cavidade alveolar, ocasionando danos no ligamento periodontal e na polpa dentária. Seu prognóstico depende das ações imediatas ao acidente, e o reimplante é a melhor forma de devolver função e estética ao paciente. Objetivo: Comparar os três últimos protocolos da International Association of Dental Traumatology (IADT) e avaliar a evolução do tratamento, destacando a terapêutica preconizada. Material e Métodos: Foram utilizadas diretrizes da IADT publicadas nos anos 2007, 2012 e 2020 e artigos relacionados à avulsão dentária, pesquisados nas plataformas digitais Pubmed e Google Acadêmico. Uma análise de classificação dos artigos encontrados, pelo indicador Scimago Journal Rank (SJR) foi utilizada. Foram incluídos apenas estudos que constaram classificações de Q1 e Q2. Resultados: atualizações importantes em meios de armazenamento com osmolaridade fisiológica, limpeza do dente avulsionado, terapia antibiótica, tratamento do canal radicular, tratamento da superfície radicular e retirada do coágulo da cavidade alveolar foram implementadas.
