Influence of the oral health care model and of economic and geographical factors on the high risk of dental caries in school children: Spatial analysis

Resumo: The aim of this study was to analyze the spatial pattern of the distribution of dental caries in schoolchildren according to the high risk to this disease and to verify the presence of spatial dependence and its correlation with socioeconomic and geographic factors. The descriptive analysis was performed from the distribution of data in thematic maps and to identify spatial dependence, the Moran Global and Local indexes were used. The multivariate linear regression technique was performed with the oral health care model, distance to the nearest Health Unit and possession of social benefit, to verify which of the independent variables best describe the occurrence of the disease. The high risk of dental caries did not show spatial autocorrelation. However, it can be seen that large concentrations were randomly distributed throughout the municipality. In the regression analysis, a statistically significant difference was observed, conditioned by the distance in meters from the central point of the census sector to the nearest Health Unit (p = 0,003). It was not possible to establish a spatial autocorrelation of the high risk of dental caries. However, the presence of a statistically significant difference related to distance, demonstrates the importance of a good allocation of the health care point, to improve access and offer equitable assistance to citizens.

Así, se realizó un estudio ecológico con datos secundarios. Se realizó un análisis descriptivo a partir de la distribución de datos en mapas temáticos y para identificar la dependencia espacial se utilizaron índices Moran Global y Local. Se utilizó la técnica de regresión lineal multivariante con el modelo de atención en salud bucal, distancia a la Unidad de Salud más cercana y posesión de beneficios sociales, para verificar cuál de las variables independientes describe mejor la ocurrencia de la enfermedad. El alto riesgo de caries dental no mostró autocorrelación espacial. Sin embargo, se puede observar que las grandes concentraciones se distribuyeron aleatoriamente por toda la ciudad. En el análisis de regresión se observó una diferencia estadísticamente significativa condicionada por la distancia en metros desde el punto central del sector censal hasta la Unidad de Salud más cercana (p = 0,003). No fue posible establecer una autocorrelación espacial de alto riesgo a caries dental. Sin embargo, la presencia de una diferencia estadísticamente significativa relacionada con la distancia demuestra la importancia de una buena asignación del punto de atención médica para mejorar el acceso y brindar una asistencia equitativa a los ciudadanos.O objetivo deste estudo foi analisar o padrão espacial da distribuição de cárie dentária em escolares de acordo com o alto risco para esta doença e verificar a presença de dependência espacial e sua correlação com fatores socioeconômicos e geográficos.  Desta forma, foi realizado um estudo ecológico com dados secundários. A análise descritiva foi realizada a partir da distribuição dos dados em mapas temáticos e para identificar a dependência espacial, foram utilizados os índices de Moran Global e Local. Realizou-se a técnica de regressão linear multivariada com o modelo de atenção à saúde bucal, distância até a Unidade de Saúde mais próxima e posse de benefício social, para verificar quais das variáveis independentes melhor descrevem a ocorrência da doença. O alto risco a cárie dentária não apresentou autocorrelação espacial. Entretanto, pode-se observar que as grandes concentrações eram distribuídas aleatoriamente por todo o município. Na análise de regressão, observou-se diferença estatisticamente significante condicionada a distância em metros do ponto central do setor censitário até a Unidade de Saúde mais próxima (p=0.003). Não foi possível estabelecer uma autocorrelação espacial do alto risco a cárie dentária.
