Intervention proposal to standardize patient restraint in the intensive care unit of a public hospital: An experience report

Resumo: | The aim of this report is to narrate the experience of a nursing resident in conducting an intervention project to standardize the process of restraining patients in an Intensive Care Unit (ICU). This descriptive, qualitative study adopts the experience report approach and covers the development of an intervention proposal from March to December 2023, based on the PDCA methodology. Data collection involved brainstorming with the participation of the multi-professional team, answering the question: "What are the obstacles to systematizing patient restraint in the ICU?" The answers, analyzed using project management tools such as the Ishikawa Diagram, GUT Matrix and 5W2H Matrix, formed the basis for the results presented in this report. The results include the identification, grouping and prioritization of the causes of the problem, culminating in the proposed intervention with action plans to address the eleven root causes that prevent the systematization of restraint in the ICU. Challenges were foreseen during implementation, such as employee resistance, limited financial resources and the possible discontinuation of the project over time. In conclusion, the successful construction of this proposal highlights the practical applicability of management tools and their substantial contributions to problem-solving. The resident acquired technical and behavioral skills, demonstrated leadership ability and developed an in-depth understanding of these tools throughout the experience. |

Este estudio descriptivo y cualitativo adopta el enfoque de informe de experiencia y abarca el desarrollo de una propuesta de intervención desde marzo hasta diciembre de 2023, basada en la metodología PDCA. La recogida de datos consistió en una lluvia de ideas con la participación del equipo multiprofesional, respondiendo a la pregunta: "Cuáles son los obstáculos para sistematizar la contención de pacientes en la UCI?" Las respuestas, analizadas mediante herramientas de gestión de proyectos como el Diagrama de Ishikawa, la Matriz GUT y la Matriz 5W2H, constituyeron la base de los resultados presentados en este informe. Los resultados incluyen la identificación, agrupación y priorización de las causas del problema, culminando en la propuesta de intervención con planes de acción para abordar las once causas fundamentales que impiden la sistematización de la contención en la UCI. Se previeron desafíos durante la implementación, como la resistencia de los empleados, los recursos financieros limitados y la posible interrupción del proyecto con el tiempo. En conclusión, el éxito de la construcción de esta propuesta pone de manifiesto la aplicabilidad práctica de las herramientas de gestión y su importante contribución a la resolución de problemas. El residente adquirió competencias técnicas y de comportamiento, demostró capacidad de liderazgo y desarrolló un profundo conocimiento de estas herramientas a lo largo de la experiencia.O objetivo deste relato é narrar a experiência de uma residente de enfermagem ao conduzir um projeto de intervenção com o propósito de padronizar o processo de contenção de pacientes em uma Unidade de Terapia Intensiva (UTI). Este estudo descritivo, de natureza qualitativa, adota a abordagem de relato de experiência e abrange o desenvolvimento de uma proposta de intervenção no período de março a dezembro de 2023, fundamentada na metodologia PDCA. A coleta de dados envolveu um Brainstorming com a participação da equipe multiprofissional, respondendo à pergunta: "Quais são os obstáculos para a sistematização da contenção de pacientes na UTI?" As respostas, analisadas por meio de ferramentas de gestão de projetos, como Diagrama de Ishikawa, Matriz GUT e Matriz 5W2H, constituíram a base para os resultados apresentados neste relato. Os resultados incluem a identificação, agrupamento e priorização das causas do problema, culminando na proposta de intervenção com planos de ação para abordar as onze causas- raiz que impedem a sistematização da contenção na UTI. Enfrentar desafios durante a implementação, como a resistência dos colaboradores, a limitação de recursos financeiros e a possível descontinuidade do projeto ao longo do tempo, foi previsto. Em conclusão, a construção bem-sucedida desta proposta destaca a aplicabilidade prática das ferramentas de gestão e suas contribuições substanciais na resolução de problemas.
