Clinical-epidemiological profile of hospitalization cases by COVID-19 in the ninth health region of Paraíba, Brazil

Resumo: Objective: To describe the clinical-epidemiological profile of cases of hospitalization due to COVID-19 in the ninth health region of Paraíba. Methodology: Descriptive observational study, with a qualitative approach, carried out at the Hospital Regional de Cajazeiras. The sample consisted of 100 medical records for the years 2020 and 2021, filed at the Hospital Epidemiological Surveillance Center of the aforementioned hospital. The medical records of individuals who were hospitalized at the HRC with a confirmed diagnosis of COVID-19 were included, and those with incomplete data and/or those who were not part of the ninth health region of Paraíba were excluded. Data collection took place from September 27th to October 8th, 2021. Results: There was an equitable division of the sample of COVID-19 cases between the years 2020 and 2021. The municipalities with the highest number of cases were: Cajazeiras (39%), Sousa (13%), São José de Piranhas (10%) and São João do Rio do Peixe (9%). There was a slight predominance of males (55%), age >60 years (55%), mixed race (69%) and ignored education (47%) or elementary school (21%). The most common risk factors were: Chronic cardiovascular diseases, including Hypertension (52%), Diabetes (24%), Smoking (15%) and Obesity (14%). Among the symptoms, the most frequent were: cough (80%), dyspnea (79%), respiratory distress (75%), low oxygen saturation (64%) and fever (61%). As for the clinical profile, most patients were not vaccinated, and of those who were, 14% received 2 doses. The most used vaccine was Coronavac/Butantan. 61% of admissions were in the COVID Ward, 58% used non-invasive ventilatory support. The mortality rate was 34%, among elderly people admitted to the ICU with invasive ventilatory support. No statistically significant results were found for the association between case evolution and risk factors. Conclusion: The cases hospitalized by COVID-19 in Paraíba presented a clinical and epidemiological profile consistent with expectations.

La muestra estuvo conformada por 100 expedientes médicos de los años 2020 y 2021, archivados en el Centro de Vigilancia Epidemiológica Hospitalaria del referido hospital. Se incluyeron las historias clínicas de los individuos que fueron hospitalizados en el HRC con diagnóstico confirmado de COVID-19 y se excluyeron aquellos con datos incompletos y / o aquellos que no formaban parte de la novena región sanitaria de Paraíba. La recolección de datos tuvo lugar del 27 de septiembre al 8 de octubre de 2021. Resultados: Hubo una división equitativa de la muestra de casos de COVID-19 entre los años 2020 y 2021. Los municipios con mayor número de casos fueron: Cajazeiras (39%), Sousa (13%), São José de Piranhas (10 %) y São João do Rio do Peixe (9%). Hubo un ligero predominio del sexo masculino (55%), edad >60 años (55%), mestizo (69%) y educación ignorada (47%) o primaria (21%). Los factores de riesgo más comunes fueron: Enfermedades cardiovasculares crónicas, incluyendo hipertensión (52%), diabetes (24%), tabaquismo (15%) y obesidad (14%). Entre los síntomas, los más frecuentes fueron: tos (80%), disnea (79%), dificultad respiratoria (75%), baja saturación de oxígeno (64%) y fiebre (61%). En cuanto al perfil clínico, la mayoría de los pacientes no fueron vacunados, y de los que sí, el 14% recibió 2 dosis. La vacuna más utilizada fue Coronavac/Butantan. El 61% de los ingresos fueron en la Sala COVID, el 58% utilizó soporte ventilatorio no invasivo. La tasa de mortalidad fue del 34%, entre los ancianos ingresados ​​en UCI con soporte ventilatorio invasivo. No se encontraron resultados estadísticamente significativos para la asociación entre la evolución de los casos y los factores de riesgo. Metodologia: Estudo observacional descritivo, de abordagem qualitativa, realizado no Hospital Regional de Cajazeiras. A amostra foi composta por 100 prontuários referentes aos anos de 2020 e 2021, arquivados no Núcleo de Vigilância Epidemiológica Hospitalar do referido hospital. Foram incluídos os prontuários dos indivíduos que passaram por internação no HRC com diagnóstico confirmado de COVID-19, e excluídos aqueles com dados incompletos e/ou que não faziam parte da nona região de saúde da Paraíba. A coleta dos dados ocorreu de 27 de setembro a 08 de outubro de 2021. Resultados: Houve divisão equitativa da amostra dos casos de COVID-19 entre os anos de 2020 e 2021. Os municípios com maior número de casos foram: Cajazeiras (39%), Sousa (13%), São José de Piranhas (10%) e São João do Rio do Peixe (9%). Houve uma discreta predominância do sexo masculino (55%), idade >60 anos (55%), raça/cor parda (69%) e ensino ignorado (47%) ou fundamental (21%). Os fatores de risco mais comuns foram: Doenças cardiovasculares crônicas, incluindo a Hipertensão (52%), Diabetes (24%), Tabagismo (15%) e Obesidade (14%). Dentre os sintomas, os mais frequentes foram: tosse (80%), dispneia (79%), desconforto respiratório (75%), baixa saturação de oxigênio (64%) e febre (61%). Quanto ao perfil clínico, a maioria dos pacientes não estavam vacinados, e dos que estavam 14% receberam 2 doses. A vacina mais usada foi a Coronavac/Butantan. 61% das internações foram na Ala COVID, 58% utilizaram suporte ventilatório não invasivo. A taxa de mortalidade foi de 34%, de pessoas idosas internadas na UTI com suporte ventilatório invasivo. Não foram encontrados resultados estatisticamente significativos para associação entre evolução do caso e fatores de risco.
