Doctor-Patient Relationship: the challenges in including transgender and transgender people in the SUS

Resumo: | Objective: To raise reflection and understand the difficulties faced by transgender and transgender people, a marginalized and stigmatized population that has suffered since childhood with symbolic and physical violence resulting in a social and psychological vulnerability in the Unified Health System (SUS). Methodology: It is a narrative review of the literature, which aims at the interpretation, reflection and analysis of the subject, of a qualitative nature. An online search was carried out in the SCIELO, PUBMED AND LILACS databases, which were restricted in the year 2009 to 2020, finding 16 articles after inclusion and exclusion critério. Bibliographic review: After researching the articles, it was noticed that most of the articles that correlated transsexuality and medical care speak of pathologies related to social characterization and the stigma of prostitution and sexually transmitted infections that this sexual and gender minority carries. In addition, it was observed difficulty in the treatment in an adequate way to make the patient comfortable, and, with that, build for a good doctor-patient relationship, which influences so that there are gaps in the attendance and consequently a deficient anamnesis, as one must take into account the psychological, social and environmental framework for a good consultation. Final considerations: Combating prejudice so that there is an improvement in the doctor-patient relationship and a more skilled system for receiving transsexuals avoiding the evasion of this population, which needs medical attention and monitoring, is urgent. |

Objetivo: Suscitar la reflexión y comprender las dificultades que enfrentan las personas transgénero y transgénero, una población marginada y estigmatizada que ha sufrido desde la niñez con violencia simbólica y física resultando en una vulnerabilidad social y psicológica en el Sistema Único de Salud (SUS). Metodología: Se trata de una revisión narrativa de la literatura, que tiene como objetivo la interpretación, reflexión y análisis del tema, de carácter cualitativo. Se realizó una búsqueda en línea en las bases de datos SCIELO, PUBMED Y LILACS, las cuales fueron restringidas en el año 2009 a 2020, encontrando 16 artículos después de los criterios de inclusión y exclusión. Revisión bibliográfica: Luego de investigar los, se notó que la mayoría de los artículos que correlacionan transexualidad y atención médica hablan de patologías relacionadas con la caracterización social y el estigma de la prostitución y las infecciones de transmisión sexual que esta minoría sexual y de género lleva. Además, se observa dificultad en el tratamiento de manera adecuada para que el paciente se sienta cómodo y, con ello, construir una buena relación médico-paciente, lo que influye para que existan vacíos en la asistencia y en consecuencia una anamnesis deficiente, como se debe tomar. tener en cuenta el marco psicológico, social y ambiental para una buena consulta. Consideraciones finales: Es urgente combatir los prejuicios para que haya una mejora en la relación médico-paciente y un sistema más habilidoso de acogida de transexuales evitando la evasión de esta población, que necesita atención médica y seguimiento.Objetivo: Suscitar reflexão e compreender as dificuldades enfrentadas por pessoas transgêneros e transexuais, população marginalizada e estigmatizada que sofre desde sua infância com a violência simbólica e física acarretando uma vulnerabilidade social e psicológica no sistema único de saúde (SUS). Metodologia: Trata-se de uma revisão narrativa da literatura, que visa a interpretação, reflexão e análise do assunto, de natureza qualitativa, foi realizada uma busca online nas bases de dados SCIELO, PUBMED E LILACS que se restringiam no ano de 2009 a 2020 encontrando 16 artigos depois dos critérios de inclusão e exclusão. Revisão bibliográfica: Após pesquisar os artigos, foi percebido que grande parte dos artigos que correlacionavam transexualidade e atenção médica falam de patologias relacionadas à caracterização social e ao estigma de prostituição e de infecções sexualmente transmissíveis que essa minoria sexual e de gênero carrega. Além disso, observou-se dificuldade no tratamento de maneira adequada para deixar o paciente confortável, e, com isso construir para uma boa relação médico-paciente, o que influencia para que haja hiatos no atendimento e consequentemente uma anamnese deficitária, pois se deve levar em conta o quadro psicológico, social e ambiental para uma boa consulta.
