Energization of irrigation solution as a supplement in the cleaning and disinfection of the root channel system

Resumo: Endodontic treatment aims to control the microbial factor to ensure the maintenance of teeth in the oral cavity in the face of the diagnosis of a diseased condition. An unfavorable factor is the anatomical complexity of the root canal system, due to the presence of apical ramifications, accessory canals and isthmus, making it difficult to completely remove organic and inorganic remnants in these areas of difficult access during instrumentation. Seeking to improve this, during endodontic treatment, one of the tools used to combat microorganisms is the irrigating solution. Still, the use of these solutions is not sufficient to remove mineral material when used individually. Thus, the help of other agents and techniques is necessary for the best result. Passive ultrasonic irrigation is based on the principle that the passive insertion of a metallic tip/file attached to an ultrasonic device in a root canal filled with an irrigating or chelating solution, when activated, allows increasing the performance of the solution agitation and improving the removal of debris, smear layer and root canal filling materials. This work aimed to carry out a narrative review of the literature addressing the effectiveness of energizing the irrigating solution after the biomechanical process of the root canal system in different protocols. In general, the works presented in this literature review showed that the methods with agitation of the irrigating solution promoted better cleaning of the canal, the isthmus and removal of the smear layer in relation to the conventional method.

Un factor desfavorable es la complejidad anatómica del sistema de conductos radiculares, debido a la presencia de ramificaciones apicales, conductos accesorios e istmos, lo que dificulta la remoción completa de remanentes orgánicos e inorgánicos en estas áreas de difícil acceso durante la instrumentación. Buscando mejorar esto, durante el tratamiento de endodoncia, una de las herramientas utilizadas para combatir los microorganismos es la solución irrigante. Aún así, el uso de estas soluciones no es suficiente para eliminar el material mineral cuando se usan individualmente. Por lo tanto, la ayuda de otros agentes y técnicas es necesaria para obtener el mejor resultado. La irrigación ultrasónica pasiva se basa en el principio de que la inserción pasiva de una punta/lima metálica unida a un dispositivo ultrasónico en un conducto radicular lleno de una solución irrigante o quelante, cuando está activada, permite aumentar el rendimiento de la agitación de la solución y mejorar la eliminación. de desechos, capa de barrillo dentinario y materiales de obturación de conductos radiculares. Este trabajo tuvo como objetivo realizar una revisión narrativa de la literatura que aborda la efectividad de energizar la solución de irrigación después del proceso biomecánico del sistema de conductos radiculares en diferentes protocolos. En general, los trabajos presentados en esta revisión bibliográfica demostraron que los métodos con agitación de la solución de irrigación promovieron una mejor limpieza del conducto, del istmo y de la remoción del barrillo dentinario en relación al método convencional.O tratamento endodôntico objetiva controlar o fator microbiano para garantir a manutenção dos dentes na cavidade bucal diante do diagnóstico de uma condição doente. Um fator desfavorável é a complexidade anatômica do sistema de canais radiculares, devido a presença de ramificações apicais, canais acessórios e istmos, dificultando a remoção completa de remanescentes orgânicos e inorgânicos nessas áreas de difícil acesso durante a instrumentação. Buscando melhorar isso, durante o tratamento endodôntico, uma das ferramentas utilizadas para o combate aos microrganismos é a solução irrigadora. Ainda assim, o uso dessas soluções não é suficiente para remoção de material mineral quando usado individualmente. Dessa forma, o auxílio de outros agentes e técnicas faz-se necessário para o melhor resultado. A irrigação ultrassônica passiva se fundamenta no princípio de que a inserção passiva de uma ponta/lima metálica acoplada a um aparelho ultrassônico em um canal radicular preenchido com uma solução irrigante ou quelante, quando ativado, permite ampliar o desempenho da agitação da solução e melhorar a remoção de detritos, smear layer e materiais de obturação do canal radicular. Este trabalho objetivou realizar uma revisão narrativa da literatura abordando a eficácia da energização da solução irrigadora após o processo biomecânico do sistema de canais radiculares em diferentes protocolos.
