Temporomandibular joint impairment in a patient with juvenile idiopathic arthritis: case report

Resumo: Objective: This article aims to present a case report of a patient with Juvenile Idiopathic Arthritis (JIA), with orofacial sequelae and impairment of the temporomandibular joint (TMJ). Case report: Patient HMM, 26 years old, female, Caucasian, attended the Occlusion, Temporomandibular Disorders, and Orofacial Pain Clinic service at UFPB with a complaint of facial pain and limited mouth opening. She reported having JIA, limited mouth opening for more than 2 months, joint sounds, using muscle relaxants, and a history of intensive use of NSAID selective COX-2 inhibitor. The extraoral examination revealed opening limitation (22 mm), with deflection to the right side, without symptoms for muscle palpation and TMJ auscultation, but with lateral sensitivity on the right side during TMJ palpation. There was an alteration in the occlusion in the position of maximum habitual intercuspation, with mild occlusal instability. The patient underwent TMJ tomography and serological tests and, after clinical and laboratory evaluation, the suggestive diagnosis was JIA with involvement of the Right TMJ and possible displacement of the articular disc without reduction in the same TMJ. The treatment consisted of counseling, associated with Occlusal Plaque, physiotherapy of compresses and stretching and rheumatological follow-up. After 6 months, there was a significant improvement in mouth opening (22 mm to 33 mm), with moderate pain control. Conclusion: The evaluation of patients should encompass a comprehensive and systemic view, with diagnosis and treatment performed in a multidisciplinary way, enabling their quality of life.

Relato de caso: Paciente HMM, 26 años, sexo femenino, caucásico, acudió a la Clínica de Oclusión, Trastornos Temporomandibulares y Dolor Orofacial de la UFPB con queja de dolor facial y limitación de la apertura bucal. Refirió tener AIJ, apertura bucal limitada durante más de 2 meses, ruidos articulares, uso de relajantes musculares y antecedentes de uso intenso de AINE Cox-2 selectivos. El examen extraoral reveló limitación de apertura (22 mm), con deflexión hacia el lado derecho, sin síntomas para la palpación muscular y auscultación de la ATM, pero con sensibilidad lateral del lado derecho durante la palpación de la ATM. Existía alteración de la oclusión en la posición de máxima intercuspidación habitual, con ligera inestabilidad oclusal. Al paciente se le realizó tomografía de ATM y estudios serológicos y, tras evaluación clínica y de laboratorio, el diagnóstico sugestivo fue AIJ con afectación de ATM Derecha y posible desplazamiento del disco articular sin reducción en la misma ATM. El tratamiento consistió en consejería, asociado a Placa Oclusal, fisioterapia de compresas y estiramientos y seguimiento reumatológico. Después de 6 meses, hubo una mejoría significativa en la apertura de la boca (22 mm a 33 mm), con control moderado del dolor. Conclusión: La evaluación de los pacientes debe abarcar una visión integral y sistémica, con diagnóstico y tratamiento realizados de forma multidisciplinaria, posibilitando calidad de vida.Objetivo: O presente artigo tem como objetivo apresentar um relato de caso de paciente portadora de Artrite Idiopática Juvenil (AIJ), com sequelas orofaciais e comprometimento da articulação temporomandibular (ATM). Relato de caso: Paciente HMM, 26 anos de idade, sexo feminino, leucoderma, compareceu ao serviço de Clínica de Oclusão, Disfunção Temporomandibular e Dor Orofacial da UFPB com queixa de dor facial e limitação de abertura bucal. Relatou ser portadora de AIJ, abertura bucal limitada há mais de 2 meses, histórico de sons articulares no passado, em uso de relaxante muscular e histórico de uso intenso de AINES Cox-2 seletivo. O exame extrabucal revelou limitação de abertura (22 mm), com deflexão para o lado direito, sem sintomatologia para palpação muscular e auscultação das ATMs, porém com sensibilidade lateral no lado direito durante palpação da ATM. Verificou-se alteração da oclusão em posição de máxima intercuspidação habitual, com instabilidade oclusal leve. Paciente realizou tomografia da ATM e exames sorológicos e, após avaliação clínica e laboratorial, o diagnóstico sugestivo foi de AIJ com comprometimento da ATM direita e possível deslocamento do disco articular sem redução na mesma ATM. O tratamento consistiu em aconselhamento, associado a Placa Oclusal, fisioterapia de compressas e alongamento e acompanhamento reumatológico. Após 6 meses, houve melhoria significativa na abertura bucal (22 mm para 33 mm), com controle moderado das dores.
