Anxiety, depressive symptoms, and quality of life in mothers of newborns with congenital malformations: a follow-up study during the first year after birth

Resumo: Objective: To evaluate the quality of life, anxiety, and depressive symptoms of mothers of newborns with congenital malformations admitted to the neonatal intensive care unit (NICU) and followed up during their first year of life. Methods: Longitudinal study carried out with mothers who had newborns with structural congenital malformations admitted to the NICU. Three outcomes were evaluated: (1) quality of life using the WHOQoL-BREF, (2) anxiety using the State-Trait Anxiety Inventory, and (3) depressive symptoms using the Beck Depression Inventory. Five evaluations were carried out: between 24h and 48h after the newborn's hospitalization, upon discharge from the NICU, three, six, and 12 months of life. Longitudinal changes in the scores of the outcomes of interest were analyzed using the Friedman test with post-hoc Conover assessment for multiple comparisons. Results: Sixteen mothers were followed for one year. During the follow-up period, no significant changes were observed in the quality of life scores, with the exception of the psychological domain (p = 0.023), especially when comparing admission to the NICU with values obtained 12 months after the child's birth (p = 0.006). Increased scores of trait-anxiety and state-anxiety were observed at 6 months of life in relation to the first assessment, being classified as severe in 75.0% and 87.5% of the mothers, respectively. Conclusion: Mothers of children with congenital malformations showed important changes in their psychological state. It is necessary to implement psychosocial support programs that promote the investigation and treatment of maternal mental disorders, especially during the first six months of the child's life.

Métodos: estudio longitudinal realizado con madres que tuvieron recién nacidos con malformaciones congénitas estructurales ingresadas en la UCIN. Se evaluaron tres resultados: (1) calidad de vida mediante el WHOQoL-BREF, (2) ansiedad mediante el Inventario de ansiedad State-Trait y (3) síntomas depresivos mediante el Inventario de depresión de Beck. Se realizaron cinco evaluaciones: entre las 24 y las 48 horas posteriores a la hospitalización del recién nacido, al alta de la UCIN, a los tres, seis y 12 meses de vida. Los cambios longitudinales en las puntuaciones de los resultados de interés se analizaron mediante la prueba de Friedman con evaluación post-hoc de Conover. Resultados: Se siguió a 16 madres durante un año. Durante el período de seguimiento, no se observaron cambios significativos en las puntuaciones de calidad de vida, con excepción del dominio psicológico (p = 0.023), especialmente al comparar el ingreso a la UCIN con los valores obtenidos 12 meses después del nacimiento del niño (p = 0.006). Se observaron mayores puntuaciones de ansiedad rasgo y ansiedad estado a los 6 meses de vida en relación con la primera evaluación, clasificándose como severo en el 75.0% y 87.5% de las madres, respectivamente. Conclusión: Las madres de niños con malformaciones congénitas mostraron cambios importantes en su estado psicológico. Es necesario implementar programas de apoyo psicosocial que promuevan la investigación y el tratamiento de los trastornos mentales maternos, especialmente durante los primeros seis meses de vida del niño.Objetivo: Avaliar a qualidade de vida, ansiedade e sintomas depressivos de mães de recém-nascidos com malformações congênitas admitidos na unidade de terapia intensiva (UTIN) e acompanhados no seu primeiro ano de vida. Foram avaliados três desfechos: (1) qualidade de vida através do WHOQoL-BREF, (2) ansiedade pelo Inventário de Ansiedade Traço-Estado, e (3) sintomas depressivos através do Inventário de Depressão de Beck. Cinco avaliações foram realizadas: entre 24h e 48h após o internamento do recém-nascido, na alta da UTIN e aos três, seis e 12 meses de vida da criança. As mudanças longitudinais nos escores dos desfechos de interesse foram analisadas através do teste de Friedman com avaliação post-hoc de Conover para múltiplas comparações. Resultados: Dezesseis mães foram seguidas durante um ano. Durante o período de acompanhamento, não foram observadas mudanças significativas nos escores de qualidade de vida, com exceção do domínio psicológico (p = 0.023) especialmente comparando-se a admissão com os valores obtidos 12 meses após o nascimento da criança (p = 0.006). Um aumento nos escores de ansiedade-traço e ansiedade-estado foi observado aos 6 meses de vida da criança em relação à primeira avaliação, sendo classificado como severo em 75.0% e 87.5% das mães, respectivamente. Conclusão: Mães de crianças com malformações congênitas apresentaram alterações importantes em seu estado psicológico.
