Prevalence and factors associated with mortality in fets and newborns with diagnosis of gastroschisis

Summary: Objective: To analyze the prevalence of mortality and the possible factors associated with this outcome in fetuses and newborns diagnosed with gastroschisis. Methods: This is a cross-sectional and retrospective study with data from neonates born with simple and complex gastroschisis. The outcome variable was mortality (yes/no) from gastroschisis. Independent variables included maternal and neonatal characteristics in fetuses and newborns diagnosed with gastroschisis. The variables were analyzed using multiple Poisson regression. Results: The prevalence of mortality in neonates with gastroschisis was 30.23%. Complex gastroschisis (p=0.002), not performing surgery to close the gastroschisis (p=0.000), performing five surgeries (p=0.000), performing surgery after the second day of birth (p=0.000), were the most important factors associated with increased risk of mortality. Hospitalization in a neonatal ICU (p=0.000) was associated, indicating a protective factor for neonatal mortality. Conclusions: There was a high prevalence of mortality in neonates with gastroschisis. The associated factors highlight the importance of early diagnosis of complex gastroschisis, as well as the early performance of surgery to close the gastroschisis.

Métodos: Este es un estudio transversal y retrospectivo con datos de neonatos nacidos con gastrosquisis simple y compleja. La variable resultado fue la mortalidad (sí/no) por gastrosquisis. Las variables independientes incluyeron características maternas y neonatales en fetos y recién nacidos con diagnóstico de gastrosquisis. Las variables se analizaron mediante regresión múltiple de Poisson. Resultados: La prevalencia de mortalidad en neonatos con gastrosquisis fue de 30,23%. Gastrosquisis compleja (p=0,002), no realizar cirugía para cerrar la gastrosquisis (p=0,000), realizar cinco cirugías (p=0,000), realizar cirugía después del segundo día de nacido (p=0,000), fueron los factores más importantes asociados con mayor riesgo de mortalidad. Se asoció la hospitalización en UTI neonatal (p=0,000), indicando factor protector para la mortalidad neonatal. Conclusiones: Hubo una alta prevalencia de mortalidad en neonatos con gastrosquisis. Los factores asociados destacan la importancia del diagnóstico precoz de la gastrosquisis compleja, así como la realización precoz de cirugía para el cierre de la gastrosquisis.Objetivo: Analisar a prevalência de mortalidade e os possíveis fatores associados a esse desfecho em fetos e recém-nascidos com diagnóstico de gastrosquise. Métodos: Trata-se de um estudo transversal e retrospectivo com dados de neonatos nascidos com gastrosquise simples e complexa. A variável ​​de desfecho foi a mortalidade (sim / não) por gastrosquise. As variáveis ​​independentes incluíram características maternas e neonatais em fetos e recém-nascidos com diagnóstico de gastrosquise. As variáveis ​​foram analisadas por meio de regressão de Poisson múltipla. Resultados: A prevalência de mortalidade em neonatos com gastrosquise foi de 30,23%.
