Congenital syphilis in Brazil: a reality to be faced

Resumo: The objective was to analyze by means of scientific evidence, how the congenital syphilis is dealt with in the scope of Primary Care, identifying the measures adopted in this context. An integrative literature review study was conducted using the databases of the platforms: Virtual Health Library (VHL), Online Medical Literature Search and Analysis System (MEDLINE), Latin American and Caribbean Literature in Health Sciences (LILACS) and Nursing Database (BDENF). The descriptors were used: “Congenital syphilis”; "Syphilis"; “Prenatal Assistance” connecting to the Boolean operator “AND”. In total, 340 articles were found, which when submitted to the inclusion and exclusion criteria, only 80 articles remained for analysis of the abstract. After reading the abstracts, 15 articles were selected to be included in the paper. The results of the study demonstrated that low socioeconomic conditions, absence or discontinuation of prenatal consultations of the pregnant woman and her partner, low education, inadequate diagnosis and treatment, lack of medication (benzathine penicillin), disqualified professionals and absence of compulsory notification, are factors that are directly interconnected in coping with congenital syphilis in primary care. It is concluded that the active search, with screening of the population included in this research, must be carried out, working on new health education strategies and public policies to reach mainly pregnant women who are more vulnerable to this damage, reinforcing through guidance, lectures and discussion groups the importance of adherence to prenatal care and early treatment of sexually transmitted infections such as syphilis.

Se realizó un estudio de revisión integradora de la literatura utilizando las bases de datos de las plataformas: Biblioteca Virtual en Salud (BVS), Sistema de Búsqueda y Análisis de Literatura Médica en Línea (MEDLINE), Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS) y Base de datos de enfermería (BDENF). Se utilizaron los descriptores: “Sífilis congénita”; "Sífilis"; "Asistencia prenatal" que se conecta al operador booleano "Y". En total, se encontraron 340 artículos, que al ser sometidos a los criterios de inclusión y exclusión solo quedaron 80 artículos para el análisis del resumen. Después de leer los resúmenes, se seleccionaron 15 artículos para ser incluidos en el trabajo. Los resultados del estudio demostraron que las bajas condiciones socioeconómicas, ausencia o interrupción de consultas prenatales de la gestante y su pareja, baja escolaridad, diagnóstico y tratamiento inadecuado, falta de medicación (penicilina benzatínica), profesionales descalificados y ausencia de notificación obligatoria, son factores que están directamente interconectados para hacer frente a la sífilis congénita en la atención primaria. Se concluye que se debe realizar la búsqueda activa, con cribado de la población incluida en esta investigación, trabajando en nuevas estrategias de educación en salud y políticas públicas para llegar principalmente a mujeres embarazadas que son más vulnerables a este daño, reforzando a través de orientaciones, charlas y grupos de discusión sobre la importancia de la adherencia a la atención prenatal y el tratamiento temprano de infecciones de transmisión sexual como la sífilis.Objetivou-se analisar por meio de evidências científicas, como se dá o enfrentamento da sífilis congênita no âmbito da Atenção Básica, identificando as medidas adotadas nesse contexto. Foi realizado um estudo de revisão integrativa da literatura que utilizou as bases de dados das plataformas: Biblioteca Virtual em Saúde (BVS), Sistema Online de Busca e Análise de Literatura Médica (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Base de Dados de Enfermagem (BDENF). Foram utilizados os descritores: “Sífilis Congênita”; “Sífilis”; “Assistência Pré-Natal” conectados ao operador booleano “AND”. No total foram encontrados 340 artigos, que ao serem submetidos aos critérios de inclusão e exclusão, restaram apenas 80 artigos para análise do resumo. Após a leitura dos resumos, 15 artigos foram selecionados para constar no trabalho. Os resultados do estudo demonstraram que condições socioeconômicas baixas, ausência ou descontinuação de consultas pré-natais da gestante e do seu parceiro, baixa escolaridade, diagnóstico e tratamento inadequados, falta do medicamento (Penicilina benzatina), profissionais desqualificados e ausência de notificação compulsória, são fatores que estão interligados diretamente no enfrentamento da sífilis congênita na atenção básica.
