Extraction of bichat ball in infant: case report

Resumo: Introduction: The buccal adipose body is a specialized mass composed of adipose tissue. It is a deep structure to the buccinator muscle, however in children this organ can be extruded into the oral cavity, characterizing a clinical entity called herniation of the buccal adipose body. The present study aims to report a case of prolapse of the oral fat pad after trauma. Case Report: Male participant, 6 months old, victim of a fall from his own height, admitted to the emergency service, presenting an injury in the oral cavity. There was no syncope and emesis after the trauma, without underlying pathologies, chronic use of medications or allergies. There were no bleeding or behavior changes. There was a prolapse of the buccal adipose body after 6 hours of blunt trauma. Physical examination showed preserved bone contours in the upper and middle third of the face, absence of crackling or edema in the nasal bones and absence of facial injuries. On intra-oral examination, a reddish-yellow lesion, similar to fat, was inserted into the right cheek mucosa, with a soft consistency. The adopted therapy was the excision of the fat pad under local anesthesia. Discussion: After 18 months of the procedure, the participant is without asymmetry and the mother reported that there were no complications during breastfeeding during this period. Conclusion: The present case corroborated with the literature in its clinical, etiological and demographic aspects, being treated through therapy already established in the literature, showing a good prognosis.

Es una estructura profunda del músculo buccinador; sin embargo, en los niños este órgano puede extruirse hacia la cavidad bucal, lo que caracteriza una entidad clínica llamada hernia del cuerpo adiposo bucal. El presente estudio tiene como objetivo reportar un caso de un paciente pediátrico con prolapso de la almohadilla de grasa oral después de un trauma. Caso clínico: Participante masculino, 6 meses de edad, víctima de una caída desde su propia altura, ingresado en el servicio de urgencias de un hospital público, presentando una lesión en la cavidad bucal. Hubo un prolapso del cuerpo adiposo bucal después de 6 horas de traumatismo cerrado. El examen físico mostró contornos óseos conservados en el tercio superior y medio de la cara, ausencia de crepitaciones o edemas en los huesos nasales y ausencia de lesiones faciales. En el examen intraoral se insertó una lesión de color amarillo rojizo, similar a la grasa, en la mucosa de la mejilla derecha, de consistencia blanda. La terapia adoptada fue la escisión de la almohadilla grasa con anestesia local. Discusión: Luego de 18 meses del procedimiento, la participante se encuentra sin asimetría y la madre informó que no hubo complicaciones durante la lactancia durante este período. Conclusión: El presente caso se corroboró con la literatura en sus aspectos clínicos, etiológicos y demográficos, siendo tratado mediante terapias ya establecidas en la literatura, mostrando un buen pronóstico.Introdução: O corpo adiposo bucal é uma massa especializada composta por tecido adiposo. É uma estrutura profunda ao músculo bucinador, entretanto em crianças esse órgão pode extruir para a cavidade bucal, caracterizando uma entidade clínica chamada de herniação do corpo adiposo bucal. O presente estudo objetiva relatar um caso de paciente pediátrico com prolapso do coxim adiposo bucal após trauma. Relato de Caso: Participante do sexo masculino, 06 meses de idade, vítima de queda da própria altura, admitido no serviço de emergência de Cirurgia e Traumatologia Bucomaxilofacial de um hospital público, apresentando ferimento em cavidade oral. Não houve síncope e êmese após o trauma, sem patologias de base, uso crônico de medicamentos ou alergias. Não ocorreram sangramentos ou alterações de comportamento. Houve o prolapso do corpo adiposo bucal após 6 horas do trauma contuso.  Ao exame físico apresentava contornos ósseos preservados em terço superior e médio de face, ausência de crepitação ou edema em ossos próprios nasais e ausência de ferimentos em face. Ao exame intra-oral, foi notada lesão de coloração amarelo-avermelhada, semelhante a gordura, inserida em mucosa jugal direita, de consistência macia. A terapêutica adotada foi a excisão do coxim adiposo sob anestesia local.  Discussão: Após 18 meses do procedimento, participante se encontra sem assimetria e a mãe informou que não houve complicações na amamentação durante esse período.
