Increase in life expectancy and population growth in Brazil with impact on the number of people living with chronic-degenerative diseases: challenges for the management of Alzheimer's Disease

Resumo: | Alzheimer's Disease (AD) and other dementias are chronic neurodegenerative diseases that reach a higher incidence in the older population, and these non-communicable chronic diseases may reach alarming numbers with the growth of the Brazilian population and the increase in life expectancy, since over the last decades the exponential increase in the incidence of non-communicable chronic diseases, such as AD, has been an inexorable phenomenon during the epidemiological transition experienced in the country. In fact, this demographic transition has been considered in theories of health actions in the country, however, little has been effectively done in practice, so that there are still various gaps in policies and actions in the field of public health that are aimed at meeting the needs of the population, local epidemiological needs regarding the attention focused on a complex health care network that can reach the most vulnerable elderly populations impacted by non-communicable chronic diseases, since a possible increase implies an increase in the demand for services and these services must be of quality, aiming at the success in the management of patients, the reduction of morbidity and mortality and the implementation of quality of life and a healthier aging for the patient with an associated decrease in disability-adjusted life years (DALY). In this systematic review we analyze the current case of AD in the Brazilian population, focusing on aspects related to the impacts of the demographic transition on the number of people living with this disease. |

In this systematic review we analyze the current case of AD in the Brazilian population, focusing on aspects related to the impacts of the demographic transition on the number of people living with this disease.La Enfermedad de Alzheimer (EA) y otras demencias son enfermedades neurodegenerativas crónicas que afectan más a la población anciana, y estas enfermedades crónicas no transmisibles pueden alcanzar cifras alarmantes con el crecimiento de la población brasileña y el aumento de la expectativa de vida, ya que en las últimas décadas la exponencial el aumento de la incidencia de enfermedades crónicas no transmisibles, como la DA, ha sido un fenómeno inexorable durante la transición epidemiológica vivida en el país. De hecho, esta transición demográfica ha sido contemplada en las teorías de las acciones de salud en el país, sin embargo, poco se ha hecho de manera efectiva en la práctica, por lo que aún existen enormes vacíos en las políticas y acciones en el campo de la salud pública que se orientan a atender las necesidades de la población necesidades epidemiológicas locales en cuanto a la atención enfocada en una red de atención de salud compleja que atienda las necesidades de las poblaciones adultas mayores más vulnerables impactadas por el ECD, ya que tal posible incremento implica un aumento en la demanda de servicios y que estos deben ser de calidad, visando el éxito en el manejo de los pacientes, la reducción de la morbimortalidad y la implementación de la calidad de vida y envejecimiento más saludable del paciente con disminución de los años de vida ajustados por discapacidad. En esta revisión sistemática analizamos el caso actual de la EA en la población brasileña, centrándonos en los aspectos relacionados con los impactos de la transición demográfica en el número de personas que viven con esta enfermedad.A Doença de Alzheimer (DA) e outras demências são doenças crônico-neurodegenerativas que atingem maior incidência na população mais idosa, sendo que estas doenças crônicas não-transmissíveis poderão atingir números alarmantes com o crescimento da população brasileira e o aumento da expectativa de vida, uma vez que nas últimas décadas o aumento exponencial na incidência destas doenças, tais como a DA, tem sido um fenômeno inexorável durante a transição epidemiológica vivenciada no país. De fato, esta transição demográfica tem sido considerada em teorias de ações de saúde no país, contudo, pouco efetivamente tem sido feito na prática, de modo que ainda existem enormes lacunas nas políticas e ações no âmbito da saúde pública que sejam voltadas a suprir as necessidades epidemiológicas locais quanto a atenção voltada a uma complexa rede de atenção em saúde que atenda às expectativas no cuidado com a saúde de populações idosas mais vulneráveis impactadas pelas doenças crônicas não-transmissíveis, visto que este possível aumento implica em um associado aumento na demanda por serviços e que estes devem ser de qualidade, visando o sucesso no manejo dos pacientes, a redução da morbimortalidade e a implementação de qualidade de vida e um envelhecimento mais saudável, com diminuição dos anos de vida perdidos ajustados por incapacitação (AVPAI).
