Surgical treatment of mandible fracture after transposition of the lower alveolar nerve: case report

Resumo: Currently, the posterior atrophic mandible region has become a challenge when it comes to rehabilitation using dental implants. Despite the high success rate of the ideal surgical techniques for each case, we encounter complications in the trans and postoperative period, such as: paresthesia, soft and bone tissue infections, hemorrhage, implant loss and mandibular fracture. The aim of this study was to report a case of fracture of the mandible after rehabilitation of the posterior region of the mandible with transposition of the lower alveolar nerve and installation of dental implants. The patient in the case report presented partial edentulism in the posterior mandible associated with severe vertical bone atrophy. The planning proposed to the patient was the transposition of the inferior alveolar nerve and the installation of dental implants at the same surgical time. Ten days after surgery, the patient was diagnosed with a fracture in the mandibular body region. With this case, it can be concluded that the surgical technique of transposition of the lower alveolar nerve has high complexity in its planning and execution, however, when performed correctly and with a technical team qualified to solve the complications, it acts promoting a new possibility of rehabilitation for patients with anatomical, aesthetic and functional limitations.

A pesar de la alta tasa de éxito de las técnicas quirúrgicas idóneas para cada caso, nos enfrentamos a complicaciones en el período trans y postoperatorio, como: parestesias, infecciones de tejidos blandos y óseos, hemorragia, pérdida de implantes y fractura de mandíbula. El objetivo de este estudio fue reportar un caso de fractura de mandíbula tras rehabilitación de la región posterior de la mandíbula con transposición del nervio alveolar inferior e instalación de implantes dentales. El paciente del caso en cuestión tenía edentulismo parcial en la región posterior de la mandíbula asociado a atrofia ósea vertical severa. La planificación propuesta fue realizar la transposición del nervio alveolar inferior e instalar implantes dentales en el mismo momento quirúrgico. Diez días después de la operación, el paciente fue diagnosticado con una fractura en la región del cuerpo mandibular. Con el caso, se puede concluir que la técnica quirúrgica de transposición del nervio alveolar inferior tiene alta complejidad en su planificación y ejecución, sin embargo, cuando se realiza correctamente y con un equipo técnico capacitado para solucionar las complicaciones, actúa promoviendo una nueva posibilidad de rehabilitación para pacientes con limitaciones anatómicas, estéticas y funcionales.Atualmente, a região posterior de mandíbula atrófica  tem se tornado um desafio quando se fala em reabilitação utilizando implantes dentários. Apesar do alto índice de sucesso das técnicas cirúrgicas ideais para cada caso, nos deparamos com complicações no trans e pós-operatório, tais como: parestesia, infecções de tecidos moles e ósseo, hemorragia, perda de implantes e fratura de mandíbula. Objetivou-se com esse estudo relatar um caso de fratura de mandíbula após realização de reabilitação de região posterior de mandíbula com transposição do nervo alveolar inferior e instalação de implantes dentários. O paciente do caso em questão apresentava edentulismo parcial em região posterior de mandíbula associada a uma severa atrofia óssea vertical. O planejamento proposto foi a realização da  transposição do nervo alveolar inferior e instalação de implantes dentários no mesmo momento cirúrgico. Após dez dias de pós-operatório o paciente foi diagnosticado com uma fratura em região de corpo mandibular.
