Preparation of a protocol for pediatric patients in cardiorespiratory arrest in a children's hospital

Resumo: Objective: To develop a protocol for the care of pediatric patients in Cardiorespiratory Arrest in a pediatric hospital in Fortaleza. Methodology: a methodological study. In the first stage, an integrative review was carried out with a search in the Pubmed/Medline, Latin American and Caribbean Literature in Health Sciences databases (LILACS); Scientific Electronic Library Online (SCIELO), and Google Scholar with the descriptors "cardiopulmonary resuscitation", "pediatrics" and "rapid response team", with a final sample of 08 scientific articles. The second stage was the preparation of the proposed protocol. Results: sample showed that the pediatric public, before evolving to a cardiorespiratory arrest event begins to present changes in vital signs and warning of physiological decline a few hours before the event itself, around 3 to 11 hours, and enters the main positive skills/characteristics, cited as necessary to professionals involved in cardiopulmonary resuscitation, are: communication (41%), experience (25%), proactivity (17%) and confidence (17%). Conclusion: the study has implications for clinical practice, such as defining specific functions for professionals involved in cardiopulmonary resuscitation, outside the intensive care and emergency environment, highlighting the main problematic points, thus pointing out opportunities for improvement and alignment of the performance of all professionals.

Metodología: estudio metodológico. En la primera etapa, se realizó una revisión integradora con búsqueda en las bases de datos Pubmed/Medline, Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS); Scientific Electronic Library Online (SCIELO) y Google Scholar con los descriptores "reanimación cardiopulmonar", "pediatría" y "equipo de respuesta rápida", con muestra final de 08 artículos científicos. La segunda etapa fue la preparación del protocolo propuesto. Resultados: la muestra mostró que el público pediátrico, antes de evolucionar a un evento de paro cardiorrespiratorio comienza a presentar cambios en las constantes vitales y advierte de deterioro fisiológico unas horas antes del evento en sí, alrededor de 3 a 11 horas, y entra en las principales habilidades/características positivas, citadas como necesarias para los profesionales involucrados en la reanimación cardiopulmonar, son: comunicación (41%), experiencia (25%), proactividad (17%) y confianza (17%). Conclusión: el estudio tiene implicaciones para la práctica clínica, como la definición de funciones específicas para los profesionales implicados en la reanimación cardiopulmonar, fuera del entorno de cuidados intensivos y urgencias, destacando los principales puntos problemáticos, señalando así oportunidades de mejora y alineación del desempeño de todos los profesionales.Objetivo: Elaborar um protocolo para assistência ao paciente pediátrico em Parada Cardiorrespiratória em hospital pediátrico de Fortaleza. Metodologia: estudo metodológico. Na primeira etapa foi realizada revisão integrativa com busca nas bases de dados Pubmed/Medline, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS); Scientific Electronic Library Online (SCIELO) e Google Schoolar com os descritores “reanimação cardiopulmonar”, “pediatria” e “time de resposta rápida”, com amostra final de 08 artigos científicos. A segunda etapa foi a elaboração da proposta de protocolo. Resultados: amostra evidenciou que o público pediátrico, antes de evoluir para um evento de parada cardiorrespiratória começa a apresentar alterações de sinais vitais e aviso de declínio fisiológico algumas horas antes do evento propriamente dito, em torno de 3 a 11 horas, e entre as principais habilidades/características positivas, citadas como necessárias aos profissionais envolvidos em uma reanimação cardiopulmonar, são: comunicação (41%), experiência (25%), proatividade (17%) e confiança (17%).
