Clinical-epidemiological characteristics of tuberculose and HIV coinfection in the State of Piauí, Brazil

Resumo: | Objective: To trace the epidemiological profile and clinical characteristics of tuberculosis infection in HIV-positive patients in Piauí. Methodology: This is an epidemiological, quantitative and descriptive study, based on the data collected in the Information System for Notifiable Diseases (SINAN) in Piauí, on TB/HIV co-infection between the period 2010 to 2019. We selected sociodemographic and clinical variables of the disease. The data were analyzed using simple descriptive statistics and presented in tables using absolute and relative frequencies. Results: 8,017 TB cases were reported, of which 546 had positive anti-HIV serology. The serological test was not performed in 2,507 (31.2%) of the reported cases. Cases of TB/HIV co-infection occurred predominantly in male individuals (77%), aged 20 to 39 years (56.4%), in the pulmonary clinical form (67.9%). Of the 422 patients who had an informed closure situation, the cure rate was 58.0%, 11.8% dropout, and 7.1% TB deaths. Conclusion: The study allowed to know the most prevalent epidemiological profile of the disease, evidencing the low rates of HIV serological investigation and cure, and high rates of dropouts and deaths in Piauí. It is necessary the provision of subsidies for directing new strategies for the prevention and control of TB/HIV co-infection in state health services. |

Metodología: Este es un estudio epidemiológico, cuantitativo y descriptivo, basado en los datos recopilados en el Sistema de Información para Enfermedades de Notificación (SINAN) en Piauí, sobre coinfección TB/VIH entre el período 2010 a 2019. Seleccionado Variables sociodemográficas y clínicas de la enfermedad. Los datos se analizaron utilizando estadísticas descriptivas simples y se presentaron en tablas con frecuencias absolutas y relativas. Resultados: Se reportaron 8.017 casos de TB, de los cuales 546 tuvieron serología positiva contra el VIH. La prueba serológica no se realizó en 2,507 (31.2%) de los casos reportados. Los casos de coinfección TB/VIH ocurrieron predominantemente en hombres (77%), de 20 a 39 años (56.4%), en la forma clínica pulmonar (67.9%). De los 422 pacientes que tenían una situación de cierre informada, la tasa de curación fue de 58.0%, 11.8% de abandono y 7.1% de muertes por TB. Conclusión: El estudio permitió conocer el perfil epidemiológico más prevalente de la enfermedad, evidenciando las bajas tasas de investigación serológica y curación del VIH, y las altas tasas de deserción y muerte en Piauí. Es necesario la provisión de subsidios para la dirección de nuevas estrategias para la prevención y el control de la coinfección TB/VIH en los servicios de salud estatales.Objetivo: Traçar o perfil epidemiológico e as características clínicas da infecção de tuberculose em pacientes HIV positivos no Piauí. Os dados foram analisados através de estatística descritiva simples e apresentados em tabelas por meio de frequências absolutas e relativas. Resultados: Foram notificados 8.017 casos de TB, dos quais 546 apresentaram sorologia positiva anti-HIV. O teste sorológico não foi realizado em 2.507 (31,2%) dos casos notificados. Os casos de coinfecção TB/HIV ocorreram predominantemente em indivíduos do sexo masculino (77%), faixa etária de 20 a 39 anos (56,4%), na forma clínica pulmonar (67,9%). Dos 422 pacientes que tiveram situação de encerramento informado, a taxa de cura foi de 58,0%, de abandono de 11,8% e de óbitos por TB de 7,1%. Conclusão: O estudo permitiu conhecer o perfil epidemiológico mais prevalente da doença, evidenciando as baixas taxas de investigação sorológica para HIV e de cura, e elevadas taxas de abandonos e de óbitos no Piauí.
