Avulsion fracture associated with desmite of the straight sesamoid ligament in a mare: diagnosis, treatment and prognosis

Resumo: | Injuries to the sesamoid ligaments can occur in horses practicing various modalities, such as racehorses being the most prone to alterations in these structures. In addition to the clinical examination, ultrasound and radiographic images can be used to diagnose possible changes in the sesamoid ligaments and bone structures. The purpose of this study was to report the clinical, radiographic and sonographic findings, as well as the therapy and outcome of a case of straight sesamoid ligament desmitis associated with avulsion fracture in horse. A mare, Quarter Horse, 5 years old, 450 kg, suddenly presented with severe lameness of the right forelimb. The animal presented claudication grade 4/5 (AAEP), increased temperature and pain on palpation in the palmar aspect of the quarter. On radiographic examination, a slight irregularity in the palmaroproximal aspect of the intermediate phalanx and an area of bone resorption associated with an irregularity in the flexor tuberosity of the intermediate phalanx was observed. In the ultrasonographic evaluation of the palmar region of the chin, it was observed the presence of areas of irregularity in the insertion of the middle scutum associated with the presence of bone fragments in the flexor tuberosity of the intermediate phalanx, as well as an increase in the size of the straight ligament and areas of fiber rupture. The use of phenylbutazone and rest for four months were indicated. When reassessed after two months, the animal presented claudication grade 2/5 (AAEP), with no positive response to palpation of the hip and forced flexion of the digit. In the case reported, the clinical examination associated with radiographic and ultrasound images were sufficient to determine the final diagnosis and subsequent prescription of the appropriate treatment. The therapy based on non-steroidal anti-inflammatory and rest proved to be effective in reducing clinical signs. |

Además del examen clínico, las imágenes de ultrasonido y radiográficas pueden usarse para diagnosticar posibles cambios en los ligamentos sesamoideos y las estructuras óseas. El propósito de este estudio fue informar los hallazgos clínicos, radiográficos y sonográficos, así como la terapia y el resultado de un caso de desmitis del ligamento sesamoideo recto asociado con una fractura por avulsión en um caballo. Una yegua, Quarter Horse, de 5 años, 450 kg, se presentó repentinamente con cojera severa de la extremidad anterior derecha. El animal presentaba claudicación grado 4/5 (AAEP), aumento de temperatura y dolor a la palpación en la cara palmar del cuarto. El examen radiográfico reveló una ligera irregularidad en la cara palmaroproximal de la falange intermedia y un área de reabsorción ósea asociada con una irregularidad en la tuberosidad flexora de la falange intermedia. En la evaluación ultrasonográfica de la región palmar del mentón se observó la presencia de áreas de irregularidad en la inserción del escudo medio asociado a la presencia de fragmentos óseos en la tuberosidad flexora de la falange intermedia, así como un aumento de el tamaño del ligamento recto y las zonas de rotura de fibras. Se indicó el uso de antiinflamatorio no esteroideo a base de fenilbutazona y reposo durante cuatro meses. Al ser reevaluado a los dos meses, el animal presentaba claudicación grado 2/5 (AAEP), sin respuesta positiva a la palpación de la cadera y flexión forzada del dedo. En el caso reportado, el examen clínico asociado a las imágenes radiográficas y ecográficas fueron suficientes para determinar el diagnóstico final y la posterior prescripción del tratamiento adecuado. La terapia a base de antiinflamatorio no esteroideo y reposo demostró ser eficaz en la reducción de los signos clínicos.Lesões nos ligamentos sesamoideanos podem ocorrer em equinos praticantes de várias modalidades, sendo os cavalos de corrida os mais propensos a alterações nestas estruturas. Além do exame clínico, as imagens ultrassonográficas e radiográficas podem ser utilizadas para o diagnóstico de possíveis alterações nos ligamentos sesamoideanos e estruturas ósseas. O propósito deste estudo foi relatar os achados clínicos, radiográficos e ultrassonográficos, bem como a terapia e desfecho de um caso de desmite do ligamento sesamoideano reto associado à fratura por avulsão em um equino. Uma égua, Quarto de Milha, 5 anos, 450 kg, apresentou subitamente um quadro de claudicação severa do membro torácico direito. O animal apresentava claudicação grau 4/5 (AAEP), aumento de temperatura e dor a palpação no aspecto palmar da quartela. Ao exame radiográfico, identificou-se leve irregularidade no aspecto palmaroproximal da falange intermédia e uma área de reabsorção óssea associado a irregularidade na tuberosidade flexora da falange intermédia. Na avaliação ultrassonográfica da região palmar da quartela, observou-se a presença de áreas de irregularidade na inserção do scutum médio associado a presença de fragmentos ósseos na tuberosidade flexora da falange intermédia, bem como aumento de tamanho do ligamento reto e áreas de ruptura de fibras. Indicou-se a utilização de de fenilbutazona e repouso por quatro meses. Quando reavaliada, após dois meses, a égua apresentou claudicação grau 2/5 (AAEP), sem resposta positiva a palpação da quartela e a flexão forçada do dígito. No caso relatado, o exame clínico associado as imagens radiográficas e ultrassonográficas foram suficientes para determinação do diagnóstico final e posterior prescrição do tratamento adequado.
