Analysis of the agreement on fall records between electronic health records and notifications: cohort study

Resumo: Objective: To analyze the agreement on fall records between patient progress data in electronic health records and notifications by the Incident Reporting System. Method: Retrospective cohort study conducted at a public hospital in Porto Alegre, Rio Grande do Sul, Brazil. The sample consisted of 367 patients, 441 self-reported notifications and 441 patient progress notes. Data were collected from the online annotation system WebAnno between September and December 2018. A collection instrument was developed. Data analysis was performed through descriptive statistics. Data were analyzed using the Statistical Package for the Social Sciences, SPSS Inc., Chicago, version 17.0 for Windows. Results: Among the patients, 316 had one fall and 51 had two falls or more. The study included 441 fall notifications. Of these, 43.9% were not recorded in the electronic medical records on the day of their occurrence. Regarding the record of risk assessment for falls, only three (0.7%) progress notes contained it.  More complete records were identified in the notifications as compared to the electronic health records. The results related to fall location, recorded in all notifications and in 13.8% of the progress notes, and damage degree, recorded in all notifications and in only 1.6% of the progress notes, are noteworthy. Conclusion: A gap of records for falls in electronic health records was identified. The results point to an aspect of extreme relevance in terms of communication via patients’ electronic health records, which can directly influence the planning and implementation of effective care.

Método: estudio de cohorte retrospectivo realizado en un hospital público de Porto Alegre, en Rio Grande do Sul, Brasil. Este estudio comprendió 367 pacientes, 441 notificaciones voluntarias y 441 evoluciones. La recolección de datos se realizó en la herramienta de anotación en línea WebAnno, de septiembre a diciembre de 2018. Se desarrolló un instrumento para la recolección. El análisis de los datos se realizó mediante estadística descriptiva. Los datos se analizaron en el paquete estadístico para las ciencias sociales, SPSS Inc, versión 17.0 de Chicago para Windows. Resultados: Entre los pacientes, 316 tuvieron una caída y 51 tuvieron dos o más caídas. El estudio incluyó 441 informes de caídas. De estos, el 43,9% no estaban registrados en la historia clínica electrónica el día de su ocurrencia. En cuanto a la valoración del riesgo de caídas, solo tres (0,7%) evoluciones contenían el récord. Al analizar los registros en las notificaciones y en la historia clínica electrónica, se identificaron informes más completos en las notificaciones. Destacan las variables locales de la caída, registradas en todas las notificaciones y en el 13,8% de las evoluciones; grado de daño, registrado en todas las notificaciones y solo en el 1,6% de las evoluciones. Conclusión: Se identificó la brecha en los registros de caídas en la historia clínica. Los resultados apuntan a un aspecto de extrema relevancia en la cuestión de la comunicación a través de la historia clínica del paciente, que puede implicar directamente en la planificación e implementación de una atención eficaz.Objetivo: Analisar a concordância entre as evoluções diárias nos prontuários eletrônicos dos pacientes e as notificações no Sistema Informatizado de Notificação no registro de quedas. Método: Estudo retrospectivo do tipo coorte, realizado em um hospital público de Porto Alegre, no Rio Grande do Sul, Brasil. Compuseram este estudo 367 pacientes, 441 notificações voluntárias e 441 evoluções. A coleta de dados ocorreu na ferramenta de anotações online WebAnno, de setembro a dezembro de 2018. Foi desenvolvido um instrumento para a coleta. A análise dos dados ocorreu por meio de estatística descritiva. Os dados foram analisados no Statistical Package for the Social Sciences, SPSS Inc, Chicago versão 17.0 para Windows. Resultados: Entre os pacientes, 316 tiveram uma queda e 51 tiveram duas quedas ou mais. O estudo incluiu 441 notificações de quedas. Destas, 43,9% não foram registradas no prontuário eletrônico no dia da sua ocorrência. Em relação à avaliação do risco de quedas, apenas três (0,7%) evoluções continham o registro. Quando analisados os registros nas notificações e no prontuário eletrônico, foram identificados relatos mais completos nas notificações. Destacam-se as variáveis local da queda, registrada em todas as notificações e em 13,8% das evoluções; grau do dano, registrada em todas as notificações e em apenas 1,6% das evoluções. Conclusão: Identificou-se a lacuna de registros da queda no prontuário.
