Acute post-traumatic extradural hematoma in a patient victim of being run over: Case report

Resumo: Introduction: Traumatic Brain Injury, defined as a traumatic injury involving the scalp, skull, meninges, vessels and/or brain, is among the most common types of trauma in emergency services worldwide. This traumatic injury has extradural hematoma as one of its main and most lethal complications. Objective: The work describes the medical approaches carried out in the case of a patient victim of being run over, presenting with acute post-traumatic extradural hematoma. Description: The case of an elderly man who reported pain in the left lower limb, loss of consciousness and expectoration of blood with bone fragments is described. The head tomography showed an acute left frontotemporoparietal extradural hematoma associated with a bone fracture, with mass effect and midline shift. As a result of these findings, the patient underwent a left frontotemporoparietal craniotomy to drain the EDH. In the following days, progressing satisfactorily, he was discharged from neurosurgery. Discussion: The risk factors and trauma mechanism were addressed in order to discuss the possible evolution and prognosis. The progression of the lesion was explored and evaluated through the clinical picture and tomography, which was essential for diagnosing the type of lesion, therapeutic definition and therapeutic monitoring of the case. Conclusion: TBI requires careful management due to the possibility of a rapid unfavorable evolution of its complications. This case report highlights that EDH is a serious traumatic process that requires a rapid diagnosis to better conduct the available clinical-surgical management.

Esta lesión traumática tiene como una de sus principales y más letales complicaciones el hematoma extradural. Objetivo: El trabajo describe los abordajes médicos realizados en el caso de un paciente víctima de atropello con hematoma extradural agudo postraumático. Descripción: Se describe el caso de un anciano que refirió dolor en miembro inferior izquierdo, pérdida de conciencia y expectoración de sangre con fragmentos óseos. La tomografía craneal mostró un hematoma extradural frontotemporoparietal agudo izquierdo asociado a fractura ósea, con efecto masa y desplazamiento de la línea media. Como resultado de estos hallazgos, el paciente fue sometido a una craneotomía frontotemporoparietal izquierda para drenar el HED. En los días siguientes, evolucionando satisfactoriamente, fue dado de alta de neurocirugía. Discusión: Se abordaron los factores de riesgo y el mecanismo del trauma para discutir su posible evolución y pronóstico. La progresión de la lesión fue explorada y evaluada a través del cuadro clínico y la tomografía, lo que fue fundamental para diagnosticar el tipo de lesión, definición terapéutica y seguimiento terapéutico del caso. Conclusión: El TCE requiere un manejo cuidadoso debido a la posibilidad de una rápida evolución desfavorable de sus complicaciones. Este reporte de caso resalta que HED es un proceso traumático grave que requiere de un diagnóstico rápido para realizar mejor el manejo clínico-quirúrgico disponible.Introdução: O Traumatismo Cranioencefálico, definido como uma lesão traumática que envolve couro cabeludo, crânio, meninges, vasos e/ou encéfalo, está entre os tipos de trauma mais comuns nos serviços de emergência mundiais. Essa lesão traumática possui o hematoma extradural como uma de suas principais e mais letais complicações. Objetivo: O trabalho descreve as abordagens médicas realizadas em caso de paciente vítima de atropelamento com hematoma extradural pós-traumático agudo. Descrição: É descrito o caso de um idoso que relatava dor em membro inferior esquerdo, perda de consciência e expectoração de sangue com fragmentos ósseos. A tomografia de crânio evidenciou hematoma extradural agudo frontotemporoparietal esquerdo associado à fratura óssea, com efeito de massa e desvio de linha média. Em decorrência desses achados, o paciente foi submetido a uma craniotomia frontotemporoparietal esquerda para drenagem do HEDA. Nos dias seguintes, evoluindo, satisfatoriamente, recebeu alta da neurocirurgia. Discussão: Foram abordados os fatores de risco e o mecanismo de trauma visando argumentar a respeito da possível evolução e do prognóstico. A progressão da lesão foi explorada e avaliada por meio do quadro clínico e da tomografia, a qual foi fundamental para o diagnóstico do tipo da lesão, a definição terapêutica e a monitorização terapêutica do caso. Conclusão: O TCE requer uma condução atenciosa devido a possibilidade de haver uma rápida evolução desfavorável de suas complicações.
