The use of ultrasonography as a tool to evaluate the quadriceps muscle in patients with chronic obstructive pulmonary disease: a systematic review

Resumo: Abstract: The aim of this review is to understand the feasibility and potential clinical utility of US in measurements of the quadriceps muscles in patients with COPD. A total of 217 studies were identified and after applying the inclusion criteria, 12 were selected. The data were systematically searched by two independent reviewers. Of the included articles, five evaluated the maximal voluntary contraction of the quadriceps muscle and the Transverse Section Area (TSA) measured by US, of these, 4 found a remarkably reduced strength in COPD and in one article, it was described that this reduction occurs in all grades of the disease. Moreover, the greater the thickness of the quadriceps, the greater the maximum voluntary contraction of this musculature, however, the TSA is reduced in these patients when compared to healthy individuals. Thus, it is suggested that the AST, evaluated by US, can be used to assess the presence and/or severity of musculoskeletal dysfunction in these patients. The sonographic evaluation of the quadriceps in patients with COPD may provide a safe, effective, low-cost, reliable and reproducible evaluation, allowing the identification and monitoring of peripheral muscle dysfunction in this population, even when compared with other strategies.

Se identificaron 217 estudios y, tras aplicar los criterios de inclusión, se seleccionaron 12. Los datos fueron buscados sistemáticamente por dos revisores independientes. De los artículos incluidos, cinco evaluaron la contracción voluntaria máxima del músculo cuádriceps y el área de la sección transversal (TSA) medida por US, de estos, 4 encontraron una fuerza notablemente reducida en la EPOC y en un artículo, se describió que esta reducción ocurre en todos los grados de la enfermedad. Además, cuanto mayor es el grosor del cuádriceps, mayor es la contracción voluntaria máxima de esta musculatura, sin embargo, la TSA está reducida en estos pacientes en comparación con los individuos sanos. Por lo tanto, se sugiere que el AST, evaluado por US, puede ser utilizado para evaluar la presencia y/o la gravedad de la disfunción musculoesquelética en estos pacientes. La evaluación ecográfica del cuádriceps en pacientes con EPOC puede proporcionar una evaluación segura, eficaz, de bajo coste, fiable y reproducible, permitiendo la identificación y el seguimiento de la disfunción muscular periférica en esta población, incluso en comparación con otras estrategias.Objetivo desta revisão é compreender a viabilidade e potencialidade da utilidade clínica da ultrassonografia (US) em medições da musculatura do quadríceps em pacientes com doença pulmonar obstrutiva crônica (DPOC). Um total de 217 estudos foram identificados e após a aplicação dos critérios de inclusão, 12 foram selecionados. Os dados foram sistematicamente buscados por dois revisores independentes. Dos artigos incluídos, cinco avaliaram a contração voluntária máxima do músculo quadríceps e a Área de Secção Transversa (AST) medida pela US, desses, 4 encontraram uma força notavelmente reduzida em DPOC e em um artigo, foi descrito que essa redução ocorre em todos os graus da doença. Além disso, quanto maior a espessura do quadríceps maior a contração voluntária máxima dessa musculatura, entretanto a AST encontra-se reduzida nestes pacientes quando comparada a indivíduos saudáveis. Com isso, sugere-se que a AST, avaliada pela US, pode ser utilizada para avaliação da presença e/ou gravidade da disfunção musculoesquelética nesses pacientes.
