Pharmacist's performance in the care of cardiac neonates

Resumo: | The insertion of the pharmacist in the multidisciplinary team aims to optimize the benefits and reduce the risks related to pharmacotherapy. The objective of this study was to describe the accomplishment of the bedside pharmaceutical follow-up for neonatal cardiac patients admitted to the Neonatal Intensive Care Unit, of a referral hospital in the state of Pará/Brazil. This is a descriptive study, with a quantitative approach, including all newborns hospitalized (19/100%), during the study period, with a closed diagnosis of congenital heart diseases, after the mother's concession and signing the Term Free and Informed Consent. For the analysis of drug interaction and incompatibility, the following databases were included: Micromedex, Drugdex and Dinamed. The types of congenital heart disease presented were interventricular communication in 6 (31.57%) neonates and 3 (15.78%) presented Tetralogy of Fallot (T4F). Almost all neonates (17/89.47%) used drugs by continuous administration in an associated way. Of these, it was observed that 8 (42.10%) received drugs that interact with each other, negatively impacting the treatment. The pharmaceutical interventions carried out were change in the medication's schedule with monitoring of possible adverse events. It is noteworthy that these actions were accepted by the service team and maintained until the medication was no longer needed and removed from the medical prescription. A computerized system, with information about the time of drug administration, could minimize pharmaceutical incompatibilities and drug interactions. It is concluded that the direct intervention of the pharmacist in the pharmacotherapeutic treatment of these patients becomes essential and can contribute to the sum of knowledge for their clinical improvement. |

El objetivo de este estudio fue describir la realización del seguimiento farmacéutico de cabecera de pacientes cardíacos neonatales ingresados en la Unidad de Cuidados Intensivos Neonatales, de un hospital de referencia en el estado de Pará / Brasil. Se trata de un estudio descriptivo, con abordaje cuantitativo, que incluye a todos los recién nacidos hospitalizados (19/100%), durante el período de estudio, con diagnóstico cerrado de cardiopatías congénitas, previa concesión de la madre y Formulario de Consentimiento Informado. Para el análisis de la interacción e incompatibilidad de medicamentos se incluyeron las siguientes bases de datos: Micromedex, Drugdex y Dinamed. Los tipos de cardiopatía congénita que se presentaron fueron comunicación interventricular en 6 (31,57%) neonatos y 3 (15,78%) presentaron Tetralogía de Fallot (T4F). Casi todos los neonatos (17 / 89,47%) utilizaron fármacos por administración continua de forma asociada. De estos, se observó que 8 (42,10%) recibieron fármacos que interactúan entre sí, impactando negativamente el tratamiento. Las intervenciones farmacéuticas realizadas fueron cambio de horario de medicación con seguimiento de posibles eventos adversos. Es de destacar que estas acciones fueron aceptadas por el equipo de servicio y se mantuvieron hasta que el medicamento dejó de ser necesario y fue retirado de la receta médica. Un sistema computarizado, con información sobre el momento de la administración del medicamento, podría minimizar las incompatibilidades farmacéuticas y las interacciones entre medicamentos. Se concluye que la intervención directa del farmacéutico en el tratamiento farmacoterapéutico de estos pacientes se vuelve fundamental y puede contribuir a la suma de conocimientos para su mejora clínica.A inserção do farmacêutico na equipe multiprofissional visa à otimização dos benefícios e redução dos riscos relacionados à farmacoterapia. Objetivou-se descrever a realização do acompanhamento farmacêutico beira-leito à pacientes neonatos cardiopatas internados em Unidade de Terapia Intensiva Neonatal, de um hospital de referência do estado do Pará/Brasil. Trata-se de um estudo de caráter descritivo, com abordagem quantitativa, sendo incluídos todos os recém nascidos internados (19/100%), no período do estudo, com diagnóstico fechado de cardiopatias congênitas, após concessão da mãe e assinatura do Termo de Consentimento Livre e Esclarecido. Para a análise de interação e incompatibilidade medicamentosa, foram incluídas as seguintes bases de dados: Micromedex, Drugdex e Dinamed. Os tipos de cardiopatias congênitas apresentadas foram comunicação interventricular em 6 (31,57%) neonatos e 3 (15,78%) apresentaram Tetralogia de Fallot (T4F). Quase que a totalidade dos neonatos (17/89,47%) utilizavam medicamentos por administração contínua de forma associada. Destes, observou-se que 8 (42,10%) receberam medicamentos que interagem entre si, impactando negativamente no tratamento. As intervenções farmacêuticas realizadas foram alteração no aprazamento do medicamento, com monitoração dos possíveis eventos adversos. Destaca-se que essas ações foram aceitas pela equipe do serviço e mantidas até que o medicamento não fosse mais necessário e retirado da prescrição médica. Um sistema informatizado, com informações à cerca do horário da administração do medicamento, poderia minimizar as incompatibilidades farmacêuticas e interações medicamentosas.
