Application of the hepatitis B vaccine in the ventrogluteal region: Evaluation of the pain reaction in newborns

Resumo: Purporse: To analyze the pain reaction produced by the application of the Hepatitis B vaccine in the ventrogluteal region in newborns and to verify its relationship with sex, gestational age of birth and anthropometric characteristics. Methodology: Quantitative, exploratory, cross-sectional study, carried out in the joint accommodation of a university hospital with a sample of 121 newborns. For the evaluation of the pain reaction, the Neonatal Pain Scale, heart rate and crying time were used. The scores produced by the scale were associated with the variables: sex, hours of life, gestational age and anthropometric measurements. During data analysis, the measures were calculated: minimum, maximum, median, mean, standard deviation, Spearmam's correlation coefficient and the Wilcoxon, Mann Whitney and Kruskal Wallis test was used. Results: Most presented pain, with a mean score of Neonatal Pain Scale 2.87 (absence of pain) before and 6.14 (presence of pain) during the procedure, being statistically significant (p <0.0001). A weak positive correlation was identified between heart rate and Neonatal Pain Scale scores and a strong correlation of the latter with crying time. Conclusion: The pain reaction is a reality even with the use of the ventrogluteal region and there is no variation according to the characteristics studied of the newborns.

Metodología: Estudio cuantitativo, exploratorio, transversal, realizado en el alojamiento conjunto de un hospital universitario con una muestra de 121 recién nacidos. Para la evaluación de la reacción al dolor se utilizó la Neonatal Pain Scale, la frecuencia cardíaca y el tiempo de llanto. Las puntuaciones producidas por la escala se asociaron a las variables: sexo, horas de vida, edad gestacional y medidas antropométricas. Durante el análisis de los datos se calcularon las medidas: mínimo, máximo, mediana, media, desviación estándar, coeficiente de correlación de Spearmam y se utilizó la prueba de Wilcoxon, Mann Whitney y Kruskal Wallis. Resultados: La mayoría presentó dolor, con una puntuación media en la Neonatal Pain Scale de 2,87 (ausencia de dolor) antes y 6,14 (presencia de dolor) durante el procedimiento, siendo estadísticamente significativa (p <0,0001). Se identificó una débil correlación positiva entre la frecuencia cardíaca y las puntuaciones de la Escala de dolor neonatal y una fuerte correlación de esta última con el tiempo de llanto. Conclusión: La reacción de dolor es una realidad incluso con el uso de la región ventroglútea y no varía según las características estudiadas de los recién nacidos.Objetivo: Analisar a reação álgica produzida pela aplicação da vacina Hepatite B na região ventroglútea em recém-nascidos e verificar sua relação com o sexo, a idade gestacional de nascimento e as características antropométricas. Os escores produzidos pela escala foram associados às variáveis: sexo, horas de vida, idade gestacional e medidas antropométricas.  Durante a análise dos dados, calculou-se as medidas: mínimo, máximo, mediana, média, desvio padrão, coeficiente de Correlação de Spearmam e utilizou-se o teste Wilcoxon, Mann Whitney e Kruskal Wallis. Resultados: A maioria dos recém-nascidos apresentou dor, com um escore médio pela Neonatal Pain Scale de 2,87 (ausência de dor) antes e 6,14 (presença de dor) durante o procedimento, sendo estatisticamente significativo (p<0,0001). Identificou-se uma correlação positiva fraca entre a frequência cardíaca e os escores da escala de Neonatal Pain Scale e uma correlação forte deste último com o tempo de choro.
