Oral surgery in patients using oral anticoagulant drugs and warfarin

Resumo: Surgical procedures are in the daily life of the dental surgeon and due to the longer maintenance of natural teeth by patients in relation to age, oral surgeries are becoming increasingly common in patients who use oral anticoagulants, so the dental surgeon must stay aware of the actions to be taken. This study aims to review the literature on the conduct to be taken in oral surgeries in patients who use anticoagulants continuously, evaluating the risk of discontinuing this medication and the risk of bleeding. For the construction of this work, a bibliographic survey was carried out in the databases SciVerse Scopus, Scientific Electronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) and ScienceDirect, using the Mendeley reference manager. The results of this research reveal that the continuation of drug therapy with warfarin in patients undergoing simple and atraumatic tooth extractions can be performed as long as the patient is in the appropriate therapeutic range analyzed through the INR. Regarding the extraction of invasive third molars, the procedure can be performed when the INR is up to 2.11, and your medical history is evaluated in detail, but more studies are needed for this type of procedure.

Este estudio tiene como objetivo revisar la literatura sobre la conducta a seguir en cirugías orales en pacientes que usan anticoagulantes de forma continua, evaluando el riesgo de discontinuar esta medicación y el riesgo de sangrado. Para la construcción de este trabajo se realizó un levantamiento bibliográfico en las bases de datos SciVerse Scopus, Scientific Electronic Library Online (Scielo), U.S. Biblioteca Nacional de Medicina (PUBMED) y ScienceDirect, utilizando el gestor de referencias de Mendeley. Los resultados de esta investigación revelan que la continuación de la terapia farmacológica con warfarina en pacientes sometidos a extracciones dentales simples y atraumáticas se puede realizar siempre que el paciente se encuentre en el rango terapéutico adecuado analizado a través del INR. En cuanto a la extracción de terceros molares invasivos, el procedimiento se puede realizar cuando el INR es hasta 2.11, y se evalúa detalladamente su historial médico, pero se necesitan más estudios para este tipo de procedimiento.Procedimentos cirúrgicos estão no dia a dia do cirurgião dentista e devido ao maior tempo de manutenção dos dentes naturais pelos pacientes em relação à idade, as cirurgias orais estão sendo cada vez mais comuns em pacientes que utilizam anticoagulantes orais, dessa forma o cirurgião dentista deve ficar atento as condutas a serem tomadas. Este estudo possui como objetivo revisar a literatura acerca das condutas a serem tomadas em cirurgias orais em pacientes que fazem uso de anticoagulante de forma contínua, avaliando o risco de suspensão dessa medicação e o risco de hemorragias. Para a construção deste trabalho foi feito um levantamento bibliográfico nas bases de dados SciVerse Scopus, Scientific Eletronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) e ScienceDirect, utilizando o gerenciador de referências Mendeley. Os resultados desta pesquisa revelam que a continuação da terapia medicamentosa com varfarina em pacientes submetidos às extrações dentárias simples e atraumática pode ser realizada desde que o paciente esteja no intervalo terapêutico adequado analisado através do RNI.
