Conservative surgical treatment in young patient with unicistic ameloblastoma: case report

Resumo: The objective of this study is to present the clinical case of a 14-year-old male patient diagnosed with unicystic ameloblastoma in the posterior region of the right mandible involving dental elements 47 and 48. The lesion was discovered in a routine imaging exam for installation of an orthodontic appliance and after evaluation of the patient, who had no associated symptoms, a less invasive treatment was proposed, through biopsy, enucleation of the lesion and installation of a decompression device at the same surgical time. After confirmation through the histopathological report of ameloblastoma, the patient has been in outpatient follow-up for 2 years, presenting a satisfactory response to treatment, with bone regeneration in the area of ​​the defect and no complaints or signs of recurrence so far. Although there is no consensus on the best therapeutics for unicystic ameloblatoma, more conservative approaches have been shown to be effective in the treatment of young patients, thus allowing the maintenance of mandibular morphology and growth.

La lesión fue descubierta en un examen de imagen de rutina para instalación de un aparato de ortodoncia y luego de la evaluación del paciente, que no presentaba síntomas asociados, se propuso un tratamiento menos invasivo, mediante biopsia, enucleación de la lesión e instalación de un dispositivo de descompresión en el mismo momento quirúrgico. Tras la confirmación mediante el informe histopatológico de ameloblastoma, la paciente lleva 2 años en seguimiento ambulatorio, presentando una respuesta satisfactoria al tratamiento, con regeneración ósea en la zona del defecto y sin quejas ni signos de recidiva hasta el momento. Aunque no existe consenso sobre la mejor terapéutica para el ameloblatoma uniquístico, se ha demostrado que los enfoques más conservadores son efectivos en el tratamiento de pacientes jóvenes, permitiendo así el mantenimiento de la morfología y el crecimiento mandibular.O objetivo deste trabalho é apresentar o caso clínico de um paciente do sexo masculino, 14 anos de idade, diagnosticado com ameloblastoma unicístico em região posterior de mandíbula direita envolvendo os elementos dentários 47 e 48. A lesão foi descoberta em exame de imagem de rotina para instalação de aparelho ortodôntico e após avaliação do paciente, que não apresentava sintomatologia associada, foi proposto um tratamento menos invasivo, por meio de biópsia, enucleação da lesão e instalação de dispositivo descompressivo no mesmo tempo cirúrgico. Após a confirmação através do laudo histopatológico de ameloblastoma, o paciente permane em acompanhamento ambulatorial há 2 anos, apresentando resposta satisfatória ao tratamento, com regeneração óssea na área do defeito e sem queixas ou sinais de recidiva até o momento.
