Conservative versus surgical treatment of condylar fractures: an integrative review

Resumo: Condylar fractures are the maxillofacial fractures with greater controversy in the literature regarding forms of treatment, since, in the procedure of choice, it involves issues related to age, location and type of fracture, degree of displacement of the fractured segment, presence of other associated facial fractures, absence of teeth, restoration of dental occlusion and the time elapsed from trauma, factors that direct the choice of the type of therapeutic approach (surgical or conservative, static or functional). This study aimed to carry out an integrative review of the current literature on the treatment of condylar fractures, highlighting its indications, limitations, advantages and disadvantages. The methodology used was an integrative literature review through bibliographic research, through a search for scientific articles in the electronic database PubMed and Medline, using the keywords mandibular condyle, mandibular fractures, mandibular injuries, indexed in the period from 2015 to 2020, which dealt with cross-sectional studies, clinical and randomized trials. After the eligibility criteria, 27 articles published in English were analyzed. Studies have shown which treatment conduct brings the most safety to the intervention, presenting high reliability, allowing for predictable and rapid scientific evidence, in addition to providing treatment with lower cost and length of stay.

Este estudio tuvo como objetivo realizar una revisión integradora de la literatura actual sobre el tratamiento de las fracturas condilares, destacando sus indicaciones, limitaciones, ventajas y desventajas. La metodología utilizada fue una revisión integradora de la literatura a través de la investigación bibliográfica, mediante la búsqueda de artículos científicos en la base de datos electrónica PubMed y Medline, utilizando las palabras clave cóndilo mandibular, fracturas mandibulares, lesiones mandibulares, indexadas en el período de 2015 a 2020, que trató de estudios transversales, ensayos clínicos y aleatorizados. Después de los criterios de elegibilidad, se analizaron 27 artículos publicados en inglés. Los estudios han demostrado qué conducta de tratamiento aporta mayor seguridad a la intervención, presentando alta confiabilidad, permitiendo evidencia científica predecible y rápida, además de brindar tratamiento con menor costo y duración de la estadía.As fraturas condilares são as fraturas maxilofaciais com maior controvérsia na literatura a respeito das formas de tratamento, uma vez que, no procedimento de escolha, envolve questões relacionadas à idade, localização e tipo de fratura, grau de deslocamento do segmento fraturado, presença de outras fraturas faciais associadas, ausência de dentes, o restabelecimento da oclusão dentária e o tempo decorrido do trauma, fatores estes que direcionam a escolha do tipo de abordagem terapêutica (cirúrgica ou conservadora, estática ou funcional). Este estudo teve como objetivo principal realizar uma revisão integrativa da literatura atual sobre o tratamento das fraturas condilares, destacando suas indicações, limitações, vantagens e desvantagens. A metodologia utilizada foi uma revisão integrativa da literatura por meio de  pesquisa bibliográfica, por meio de uma busca de artigos científicos na base de dados eletrônica PubMed e Medline, utilizando os descritores mandibular condyle, mandibular fractures, mandibular injuries, indexados no período de 2015 a 2020, que tratavam de estudos transversais, ensaios clínicos e randomizados. Após os critérios de elegibilidade, foram analisados 27 artigos integralmente publicados em língua inglesa.
