Contributions of Obstetric Nursing to safe care for parturientes and newborns in the contexto of the pandemic COVID-19

Resumo: | Objectives: To discuss the current state of publications on obstetric care for asymptomatic women for COVID-19 and to point out the contributions of obstetric nursing to the safe care of parturients and newborns in the context of the pandemic. Method: Systematic review based on Cochrane, Embase, BVS; MEDLINE, PubMed, CINAHL and Scopus, to raise publications that answered the question: What are the recommended care measures for assisting asymptomatic women for COVID-19 during childbirth and the puerperium? Results: 36 studies were surveyed, but only 2 met the eligibility criteria. It was found that the publications focus on assistance to pregnant women, parturients and postpartum women symptomatic and / or positive for COVID-19. However, the reality of Brazilian health services imposes challenges for safe obstetric care due to the lack of PPE, the absence of mass testing and the architecture specificities for the maternity hospitals. These challenges may imply an increased risk of spreading the virus in these places and exposure to healthy pregnant women and health professionals. In this context, in line with the actions to combat COVID-19, we point out guidelines for respectful care for women's rights and insurance for parturients, newborns, mothers, companions and health professionals within the scope of obstetric care services. Conclusion: The set of care proposed for the differents levels of health care has the potential to contribute to obstetric nursing care in the current Brazilian epidemiological scenario, without losing sight of the importance of humanization, quality and safety. |

Método: revisión sistemática basada en Cochrane, Embase, BVS; MEDLINE, PubMed, CINAHL y Scopus, para plantear publicaciones que respondan a la pregunta: ¿Cuáles son las medidas de atención recomendadas para ayudar a las mujeres asintomáticas con COVID-19 durante el parto y el puerperio? Resultados: fueron encuestados 36 estudios, pero solo 2 cumplieron con los criterios de elegibilidad. Se encontró que las publicaciones se centran en la asistencia a mujeres embarazadas, parturientas y mujeres posparto sintomáticas y / o positivas para COVID-19. Sin embargo, la realidad de los servicios de salud brasileños impone desafíos para la atención obstétrica segura debido a la falta de EPP, la ausencia de pruebas masivas y las especificidades de la arquitectura de los hospitales de maternidad. Estos desafíos pueden implicar un mayor riesgo de propagación del virus en estos lugares y la exposición a mujeres embarazadas sanas y profesionales de la salud. En este contexto, en línea con las acciones para combatir COVID-19, señalamos pautas para la atención respetuosa de los derechos de las mujeres y el seguro para las parturientas, recién nacidos, madres, acompañantes y profesionales de la salud dentro del alcance de los servicios de atención obstétrica. Conclusión: El conjunto de cuidados propuestos para los diferentes niveles de atención de salud tiene el potencial de contribuir a la atención de enfermería obstétrica en el escenario epidemiológico brasileño actual, sin perder de vista la importancia de la humanización, la calidad y la seguridad.Objetivos: Discutir o estado atual das publicações sobre a assistência obstétrica às mulheres assintomáticas para COVID-19 e apontar as contribuições da enfermagem obstétrica para o cuidado seguro à parturientes e neonatos no contexto da pandemia. Método: Revisão sistemática nas bases Cochrane, Embase, BVS; MEDLINE, PubMed, CINAHL e Scopus, para levantar publicações que respondessem à questão: Quais são os cuidados recomendados para a assistência às mulheres assintomáticas para COVID-19 durante o parto e puerpério? Resultados: Foram levantados 36 estudos, porém apenas 2 atenderam aos critérios de elegibilidade. Constatou-se que as publicações enfocam a assistência às gestantes, parturientes e puérperas sintomáticas e/ou positivas para COVID-19. No entanto, a realidade dos serviços de saúde brasileiros impõe desafios para a atenção obstétrica segura pela falta de EPI, ausência de testagem em massa e especificidades da arquitetura das maternidades. Esses desafios podem implicar em aumento do risco de disseminação do vírus nestes locais e de exposição para gestantes saudáveis e profissionais de saúde. Nesse contexto, em consonância com as ações de enfrentamento à COVID-19, apontamos orientações para o cuidado respeitoso aos direitos femininos e seguro para parturientes, neonatos, puérperas, acompanhantes e profissionais de saúde no âmbito dos serviços da atenção obstétrica.
