Surgical approach in the treatment of temporomandibular joint ankylosis

Resumo: | Ankylosis of the temporomandibular joint (TMJ) occurs when the condyle-disk complex joins the articular surface of the temporal bone, resulting in an anatomical-functional alteration. Its etiology is multifactorial, with trauma associated with condylar fracture being the most common cause. This condition results in various functional problems, including limited mouth opening and reduced chewing capacity in affected patients. Diagnosis is established by correlating clinical and imaging findings, enabling an accurate assessment and guiding therapeutic decisions. Currently, various surgical modalities are used to treat TMJ ankylosis, including gap arthroplasty, interpositional arthroplasty, joint reconstruction and osteogenic distraction. With this in mind, this paper aims to present and analyze the different surgical interventions used to treat TMJ ankylosis, offering a concise overview of the possible forms of treatment. To develop this article, searches were carried out in the Google Scholar, PubMed and SciELO databases. The study covered works published between 2013 and 2023 in Portuguese, English and Spanish. This review highlighted the various techniques available to address TMJ ankylosis. However, controversies persist regarding the standard method of treatment, with no clear consensus among researchers. Regardless of the technique adopted, it is essential that treatment not only relieves symptoms, but also restores the functionality of the stomatognathic system and reduces the risk of reankylosis. |

Su etiología es multifactorial, siendo el traumatismo asociado a fractura condilar la causa más frecuente. Esta afección provoca varios problemas funcionales, en particular una apertura bucal limitada y una capacidad de masticación reducida en los pacientes afectados. El diagnóstico se establece mediante la correlación entre aspectos clínicos e imagenológicos, permitiendo una evaluación precisa y orientando las decisiones terapéuticas. En la actualidad, se utilizan diversas modalidades quirúrgicas para tratar la anquilosis de la ATM, como la artroplastia gap, la artroplastia interposicional, la reconstrucción articular y la distracción osteogénica. Teniendo esto en cuenta, este artículo pretende presentar y analizar las diferentes intervenciones quirúrgicas utilizadas para tratar la anquilosis de la ATM, ofreciendo una visión concisa de las posibles formas de tratamiento. Para la elaboración de este artículo se realizaron búsquedas en las bases de datos Google Scholar, PubMed y SciELO. El estudio abarcó trabajos publicados entre 2013 y 2023 en portugués, inglés y español. Esta revisión destacó las diversas técnicas disponibles para abordar la anquilosis de la ATM. Sin embargo, persisten controversias en cuanto al método estándar de tratamiento, sin que exista un consenso claro entre los investigadores. Independientemente de la técnica adoptada, es esencial que el tratamiento no sólo alivie los síntomas, sino que también restablezca la funcionalidad del sistema estomatognático y reduzca el riesgo de reanquilosis.A anquilose da articulação temporomandibular (ATM) ocorre quando o complexo disco-côndilo se une à superfície articular do osso temporal, resultando em uma alteração anatômica-funcional. Sua etiologia é multifatorial, sendo o trauma associado a fratura condilar a causa mais comum. Essa condição resulta em vários problemas funcionais, destacando-se a limitação na abertura bucal e a redução da capacidade de mastigação nos pacientes afetados. O diagnóstico é estabelecido através da correlação entre os aspectos clínicos e de imagem, possibilitando uma avaliação precisa e embasando as decisões terapêuticas. Atualmente, várias modalidades cirúrgicas são empregadas para tratar a anquilose da ATM, estas incluem artroplastia em gap, artroplastia interposicional, reconstrução articular e distração osteogênica. Nesse sentindo, o presente trabalho tem como objetivo apresentar e analisar as diferentes intervenções cirúrgicas usadas no tratamento de anquilose da ATM, oferecendo uma visão concisa das possíveis formas de tratamento.
