Care for the population in street situations: dilemmas and challenges for the implementation of street consultants

Resumo: | The present work brings a reflection on how care and health care was built for the homeless population in Brazil. It is a population surrounded by stigmas and lacking everything, for this reason this construction took place slowly and began with a strong welfare character. With the reformulation of the National Primary Care Policy, a specific model of care was instituted for the care of this population, the Consultório na Rua (CnaR), in which the teams provide care in circumstances different from those they are used to. The numerous difficulties inherent to this process are highlighted, such as the difficulty of intersectionality and the bureaucratic barriers that prevent their access to continuous care. It is concluded that the professional who works in the CnaR team needs to understand the health-disease process of these people and practice a qualified listening, with an ethical positioning, in addition, they must break the barrier of standardization to offer a humanized, singularized, qualified care, with a sensitive listening, capable of welcoming the user and his demands. |

Es una población rodeada de estigmas y carente de todo, por ello esta construcción se dio lentamente y comenzó con un fuerte carácter asistencial. Con la reformulación de la Política Nacional de Atención Primaria, se instituyó un modelo de atención específico para la atención de esta población, el Consultório na Rua (CnaR), en el que los equipos brindan atención en circunstancias diferentes a las que están acostumbrados. Se destacan las numerosas dificultades inherentes a este proceso, como la dificultad de la intersectorialidad y las barreras burocráticas que impiden su acceso a la atención continua. Se concluye que el profesional que actúa en el equipo del CnaR necesita comprender el proceso salud-enfermedad de estas personas y practicar una escucha cualificada, con un posicionamiento ético, además, debe romper la barrera de la estandarización para ofrecer una atención humanizada, singularizada. Atención calificada, con una escucha sensible, capaz de acoger al usuario y sus demandas.O presente trabalho traz uma reflexão acerca de como se construiu o cuidado e atenção à saúde para a população em situação de rua no Brasil. Trata-se de uma população cercada por estigmas e a quem tudo falta, por esse motivo essa construção se deu de forma lenta e começou com um forte caráter assistencialista. Com a reformulação da Política Nacional de Atenção Básica instituiu-se um modelo de cuidado específico para o atendimento de dita população, o Consultório na Rua (CnaR), em que as equipes realizam atendimentos em circunstâncias distintas daquelas que estão habituados. Destacam-se as inúmeras dificuldades inerentes a esse processo, tais como a dificuldade de intersetorialidade e as barreiras burocráticas que impedem o acesso destes ao cuidado contínuo.
