High power lasers in frenectomy, their benefits and limitations: integrative review

Resumo: High power surgical lasers are indicated for soft tissue surgeries of the oral cavity, such as frenectomy, which consists of the complete removal of the buccal or lingual frenulum. Objective: This integrative review aims to punctuate the types of surgical lasers, and the benefits and limitations of their use in the removal of frenulum. Methodology: Search and analysis of articles, in English, Spanish and Portuguese, indexed from 2016 to 2022, in the digital databases LILACS, PubMed, BBO, Cochrane, MEDLINE and SciELO using the descriptors “laser”, “frenectomy” and “oral surgery” with the AND association. This methodology followed the PRISMA recommendations (Key Items for Reporting Systematic Reviews and Meta-analysis), adapted for integrative review. Results: The search generated 57 articles, after inclusion and exclusion criteria, 20 articles of the literature review, systematic review, qualitative study, randomized study, case report, clinical study and meta-analysis were selected. Conclusion: Regarding the benefits of these mechanisms, the reduction in surgical time, greater cutting precision, less bleeding and pain, faster healing and greater comfort in the postoperative period, as there is no need for suture. As for the limitations highlighted, they would be the little discussion and clinical experience of the use of surgical lasers in general, and their high cost for acquisition and maintenance, which limits public and private institutions and services to acquire them. They also emphasize the need and incentive for clinical research with these equipment and focusing on this theme, as all the lasers discussed present satisfactory results, especially in the postoperative period.

Objetivo: Esta revisión integradora tiene como objetivo puntuar los tipos de láseres quirúrgicos, y los beneficios y limitaciones de su uso en la remoción del frenillo. Metodología: Búsqueda y análisis de artículos, en inglés, español y portugués, indexados de 2016 a 2022, en las bases de datos digitales LILACS, PubMed, BBO, Cochrane, MEDLINE y SciELO utilizando los descriptores “láser”, “frenectomía” y “cirugía oral” con la asociación AND. Esta metodología siguió las recomendaciones PRISMA (Key Items for Reporting Systematic Reviews and Meta-analysis), adaptadas para revisión integradora. Resultados: La búsqueda generó 57 artículos, luego de criterios de inclusión y exclusión, se seleccionaron 20 artículos de la revisión bibliográfica, revisión sistemática, estudio cualitativo, estudio aleatorizado, reporte de caso, estudio clínico y metanálisis. Conclusión: En cuanto a los beneficios de estos mecanismos, la reducción del tiempo quirúrgico, mayor precisión de corte, menor sangrado y dolor, cicatrización más rápida y mayor comodidad en el postoperatorio, ya que no hay necesidad de sutura. En cuanto a las limitaciones destacadas, serían la poca discusión y experiencia clínica del uso de los láseres quirúrgicos en general, y su alto costo de adquisición y mantenimiento, lo que limita a instituciones y servicios públicos y privados para adquirirlos. También destacan la necesidad y el incentivo de la investigación clínica con estos equipos y con foco en esa temática, ya que todos los láseres discutidos presentan resultados satisfactorios, especialmente en el postoperatorio.Os lasers cirúrgicos de alta potência estão indicados para cirurgias de tecido mole da cavidade oral, tipo frenectomia, que consiste na remoção completa do frênulo vestibular ou lingual. Objetivo: Esta revisão integrativa objetiva pontuar os tipos de lasers cirúrgicos, e os benefícios e limitações do uso na remoção dos frênulos. Metodologia: Busca e análise de artigos, em inglês, espanhol e português, indexados no período de 2016 a 2022, nas bases de dados digitais LILACS, PubMed, BBO, Cochrane, MEDLINE e SciELO utilizando os descritores “laser”, “frenectomy” e “oral surgery” com a associação AND.  Esta metodologia seguiu as recomendações PRISMA (Principais Itens para Relatar Revisões Sistemáticas e Metanálise), adaptada para revisão integrativa. Resultados: A busca gerou 57 artigos, após critérios de inclusão e exclusão, 20 artigos do tipo revisão de literatura, revisão sistemática, estudo qualitativo, estudo randomizado, relato de caso, estudo clínico e meta-análise foram selecionados. Conclusão: Quanto aos benefícios destes mecanismos destacam a diminuição no tempo cirúrgico, maior precisão de corte, menor sangramento e dor, rápida cicatrização e maior conforto no pós cirúrgico por não precisar de sutura. Quanto às limitações destacadas seriam a pouca discussão e vivência clínica da utilização dos lasers cirúrgicos de uma forma geral, e seu custo elevado para aquisição e manutenção, o que limita instituições e serviços públicos e privados adquiri-los.
