Visceral Leishmaniasis and Human Immunodeficiency Virus Coinfection: epidemiological profile of cases reported in São Luís-Maranhão, Brazil

Resumo: Visceral Leishmania is the clinical form of leishmaniasis that is most associated with HIV/AIDS. The co-infection Leishmania sp/HIV has been considered as an emerging disease in several regions of the world due to the geographical overlap of the two infections, as a consequence of the urbanization of leishmaniasis and the ruralization of HIV infection. The present work aims to study the epidemiological profile of reported cases of co-infection of visceral leishmaniasis and human immunodeficiency virus in the years 2010 to 2019 in São Luis, Maranhão State. All data were collected in November 2020 through the Acute Notification Information System (SINAN) made available on the website of the Department of Informatics of the Unified Health System (DATASUS), taking into account the variables chosen as age, education, gender, ethnicity and diagnosis. The Acute Notification Information System (SINAN) between the years 2010 and 2019 were notified 1994 cases of visceral leishmaniasis in São Luis Maranhão, of which 321 had co-infection with HIV. We can observe that between 2010 and 2019 a percentage of 83.80% reported were male, between the ages of 40-59, especially individuals of black race / color with low education. Based on the analysis we can observe that the most affected population are vulnerable people with a reduced level of education which makes them more susceptible, and action is needed to raise health awareness aimed at such population in order to direct the control of the disease and make the control more effective.

La coinfección Leishmania sp/VIH se ha considerado una enfermedad emergente en varias regiones del mundo debido al solapamiento geográfico de ambas infecciones, como consecuencia de la urbanización de la leishmaniasis y la ruralización de la infección por VIH. Este estudio tiene como objetivo estudiar el perfil epidemiológico de los casos notificados de coinfección de leishmaniasis visceral y virus de la inmunodeficiencia humana en los años 2010 a 2019 en São Luis, Estado de Maranhão. Todos los datos fueron recogidos en noviembre de 2020 a través del Sistema de Información de Notificación de Agudos (SINAN) disponible en la página web del Departamento de Informática del Sistema Único de Salud (DATASUS), teniendo en cuenta las variables elegidas como la edad, la educación, el género, la etnia y el diagnóstico. El Sistema de Información de Notificación de Agudos (SINAN) entre los años 2010 y 2019 se notificaron 1994 casos de leishmaniasis visceral en São Luis Maranhão, de los cuales 321 tenían coinfección con el VIH. Entre 2010 y 2019, un porcentaje del 83,80% de los reportados eran hombres, de entre 40 y 59 años, especialmente individuos de raza/color negra con baja educación. A partir del análisis podemos observar que la población más afectada son personas vulnerables con un nivel de educación reducido que las hace más susceptibles, por lo que es necesario actuar en la concienciación sanitaria dirigida a esta población para dirigir el control de la enfermedad y que éste sea más efectivo.A Leishmania visceral é a forma clínica das leishmanioses que está mais associada ao HIV/Aids A coinfecção Leishmania sp x HIV tem sido considerada como doença emergente em várias regiões do mundo em função da superposição geográfica das duas infecções, como consequência da urbanização das leishmanioses e da ruralização da infecção por HIV. O presente trabalho tem como objetivo de realizar estudo epidemiológico no período de 2010 a 2019 sobre infeção de leishmaniose visceral e vírus da imunodeficiência adquirida notificada na cidade de São Luís, no estado do Maranhão. Todos os dados foram coletados no mês de novembro de 2020 através do Sistema de Informações de Agravos de Notificação (SINAN) disponibilizados no site do Departamento de Informática do Sistema Único de Saúde (DATASUS). O Sistema de Informações de Agravos de Notificações (SINAN) entre os anos de 2010 é 2019 foram notificados 1994 casos de leishmaniose visceral em São Luis - Maranhão, dentre esses 321 possuíam confeição com HIV. Podemos observar que entre 2010 e 2019 uma porcentagem de 83,80% notificados são do gênero masculino, entre as faixas etária de 40-59 anos, especialmente indivíduos raça/cor preta com uma escolaridade baixa. Com base na análise podemos observar que população mais afetada são pessoas vulneráveis, com poucas instruções e os que torna mais suscetíveis, sendo necessário uma ação para conscientização dessas pessoas, uma política de saúde que abrange essa dada população.
