Factors associated with the development of acute myocardial infarction in patients with COVID-19

Resumo: | Objective: To describe the factors associated with the development of Acute Myocardial Infarction (AMI) in patients with COVID-19. Methodology: This is an integrative literature review, with the purpose of gathering and synthesizing results of investigations on the subject. For the selection of studies, PubMed (North American Biomedical Literature, created by the US National Library of Medicine), Web of Science and Scopus databases were searched, from January to March 2022. The descriptors used were: (COVID-19) OR (SARS-CoV-2) AND (MYOCARDIAL INFARCTION) AND (CORONARY THROMBOSIS). The final sample consisted of 08 analyzed articles. Results: The main factors that are associated with the occurrence of AMI in patients positive for COVID-19 are: the advanced age of the individuals, patients with high comorbidities and being male. The most common alterations in clinical and laboratory tests are the elevation of the st segment on the electrocardiogram, increased levels of troponin, D-dimer and serum C-reactive protein. The association between these factors represents a greater severity of these patients with a higher mortality rate. Conclusion: Knowledge of information regarding the baseline characteristics of cases associated with acute myocardial infarction (AMI) to patients with COVID-19 contributes to organizing the management of these patients in hospital systems through the institution of appropriate protocols for rapid diagnosis, screening and isolation. And also to minimize the exposure of the health team. |

Para la selección de estudios, se realizaron búsquedas en las bases de datos PubMed (North American Biomedical Literature, creada por la Biblioteca Nacional de Medicina de EE. UU.), Web of Science y Scopus, de enero a marzo de 2022. Los descriptores utilizados fueron: (COVID-19) O (SARS-CoV-2) Y (INFARTO DE MIOCARDIO) Y (TROMBOSIS CORONARIA). La muestra final estuvo constituida por 08 artículos analizados. Resultados: Los principales factores que se asocian a la ocurrencia de IAM en pacientes positivos para COVID-19 son: la edad avanzada de los individuos, pacientes con altas comorbilidades y ser del sexo masculino. Las alteraciones más comunes en las pruebas clínicas y de laboratorio son la elevación del segmento st en el electrocardiograma, aumento de los niveles de troponina, dímero D y proteína C reactiva sérica. La asociación entre estos factores representa una mayor gravedad de estos pacientes con una mayor tasa de mortalidad. Conclusión: El conocimiento de la información sobre las características basales de los casos asociados a infarto agudo de miocardio (IAM) a pacientes con COVID-19 contribuye a organizar el manejo de estos pacientes en los sistemas hospitalarios a través de la institución de protocolos apropiados para diagnóstico rápido, tamizaje y aislamiento. Y también para minimizar la exposición del equipo de salud.Objetivo: Descrever os fatores associados ao desenvolvimento de Infarto Agudo do Miocárdio (IAM) em pacientes com COVID -19. Metodologia: Trata-se de uma revisão integrativa da literatura, com a finalidade de reunir e sintetizar resultados de investigações sobre o tema. Para a seleção dos estudos, foram pesquisadas as bases de dados Medline/PubMed (Literatura Biomédica norte-americana, criado pela US Nacional Library of Medicine), Web of Science e Scopus, a busca foi realizada no mês de fevereiro de 2022. Os descritores utilizados foram: (COVID-19) OR (SARS-CoV-2) AND (MYOCARDIAL INFARCTION) AND (CORONARY THROMBOSIS). A amostra final consistiu em 08 artigos analisados. Resultados: Os principais fatores que estão associados à ocorrência de IAM em pacientes positivos para COVID-19, são a idade avançada dos indivíduos, pacientes com comorbidades e ser do sexo masculino. Nas alterações de exames clínicos e laboratoriais os mais comuns foram a elevação do segmento ST no eletrocardiograma, aumento dos níveis de troponina, do D-dímero e da proteína C reativa sérica. A associação entre esses fatores representa uma maior gravidade desses pacientes com alta taxa de mortalidade. Conclusão: O conhecimento de informações quanto às características basais dos casos associados Infarto agudo do miocárdio (IAM) aos pacientes com COVID-19 contribui para organizar o manejo destes pacientes no sistema hospitalar através da instituição de Protocolos apropriados para diagnóstico rápido, triagem e isolamento.
