Maternal reactions and feelings towards the hospitalization of the child in a Neonatal Intensive Care Unit: a narrative literature review

Resumo: Introduction: There is an intrinsic social pressure placed on women's lives in relation to motherhood and, therefore, when the gestation period does not end as expected, there is a great impact on the family environment and, especially, on the mother-baby relationship. Objectives: to elucidate the conceptions regarding the feeling of motherhood in the face of the hospitalization of the neonate in intensive care, seeking to establish challenges and learning related to the scope. Methodology: This is a descriptive research of the type narrative review of the literature. The research was carried out through online access to the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) and EBSCO databases. Information Services. Results and discussion: The high complexity of procedures performed in the Intensive Care Unit (NICU) restricts the visiting hours and physical contact so expected by the pregnant woman and, therefore, the hospitalization of a newborn (NB) in the neonatal ICU demands the reorganization of a new routine for the newborn's family. In this way, the family dynamics is altered and arouses several feelings in the puerperal woman, such as anxiety due to early separation, failure, insecurity, concern and decreased or even lack of confidence in the ability to care for her child. This experience lived by the mother can trigger the emergence of depression, anxiety disorders, fatigue and sleep disorders, affecting the family bond. Final considerations: With this, it is noted that the hospitalization of an NB impacts the idealized expectations of the child's birth. Thus, the communication of the hospital's multidisciplinary team with the baby's family is extremely important for the construction of the family's autonomy.

Metodología: Se trata de una investigación descriptiva del tipo revisión narrativa de la literatura. La investigación se llevó a cabo a través del acceso en línea a la Biblioteca Nacional de Medicina (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) y las bases de datos EBSCO. Servicios. Resultados y discusión: La alta complejidad de los procedimientos realizados en la Unidad de Cuidados Intensivos (UCIN) restringe los horarios de visita y el contacto físico tan esperado por la gestante y, por tanto, la hospitalización de un recién nacido (RN) en la UTI neonatal demanda la reorganización de una nueva rutina para la familia del recién nacido. De esta manera, la dinámica familiar se altera y despierta varios sentimientos en la puérpera, como ansiedad por la separación temprana, fracaso, inseguridad, preocupación y disminución o incluso falta de confianza en la capacidad de cuidar a su hijo. Esta experiencia vivida por la madre puede desencadenar la aparición de depresión, trastornos de ansiedad, fatiga y trastornos del sueño, afectando el vínculo familiar. Consideraciones finales: Con esto, se constata que la hospitalización de un RN impacta en las expectativas idealizadas del nacimiento del niño. Así, la comunicación del equipo multidisciplinario del hospital con la familia del bebé es de suma importancia para la construcción de la autonomía familiar.Introdução: Há uma pressão social intrínseca depositada sobre a vida das mulheres em relação à maternidade e, por isso, quando o período gestatório não termina como o esperado, ocorre um grande impacto no ambiente familiar e, principalmente, na relação mãe-bebê. Objetivos: elucidar as concepções referentes ao sentimento da maternidade diante da internação do neonato em terapia intensiva, procurando estabelecer desafios e aprendizados referentes ao âmbito. Metodologia: Trata-se de uma pesquisa descritiva do tipo revisão narrativa da literatura. A pesquisa foi realizada através do acesso online nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services. Resultados e discussão: A alta complexidade de procedimentos realizados na Unidade de Terapia Intensiva (UTIN) restringe os horários de visita e contato físico tão esperado pela gestante e, por isso, a internação de um recém-nascido (RN) na UTI neonatal demanda a reorganização de uma nova rotina para a família do neonato. Dessa forma, a dinâmica familiar é alterada e desperta diversos sentimentos na puérpera, como ansiedade devido a separação precoce, fracasso, insegurança, preocupação e diminuição ou até mesmo ausência de confiança na capacidade de cuidar do seu filho. Essa experiência vivenciada pela mãe pode desencadear o surgimento de quadros depressivos, transtornos de ansiedade, fadiga e distúrbios do sono afetando o vínculo familiar. Considerações finais: Com isso, nota-se que a hospitalização de um RN impacta as expectativas idealizadas diante do nascimento do filho.
