Effects of deep dry needing on the treatment of temporomandibular dysfunction: case study

Resumo: This study aimed to evaluate the effectiveness of deep dry needling for the treatment of temporomandibular disorders (TMD) through the numerical pain scale, hospital anxiety and depression scale and mouth opening. Three patients were evaluated and diagnosed with TMD by clinical examination based on Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Pain intensity was also assessed using the numerical pain scale, mouth opening measurement (with a caliper) and the Hospital Anxiety and Depression Scale (HADS) questionnaire to assess anxiety and depression. Dry needling at trigger points in the masseter and temporal muscles was performed using 0.20 x 13 mm needles. After 4 weeks of treatment, 1 session per week, the results showed that one patient had mixed TMD and the other 2 myogenous; that pain intensity and HADS questionnaire score for anxiety and depression decreased for all patients and promoted an increase in mouth opening in the patient with limitation. In conclusion, dry needling in active trigger points in the masseter and temporal muscles in patients with myofascial TMD showed important clinical improvement after 4 weeks of treatment with pain resolution and decreased anxiety and depression.

Tres pacientes fueron evaluados y diagnosticados con TMD mediante un examen clínico basado en los Criterios de diagnóstico de investigación para trastornos temporomandibulares (RDC / TMD). La intensidad del dolor también se evaluó mediante la escala numérica del dolor, la medición de la apertura de la boca (con un calibre) y el cuestionario de la Escala de ansiedad y depresión hospitalaria (HADS) para evaluar la ansiedad y la depresión. La punción seca en los puntos gatillo del masetero y los músculos temporales se realizó con agujas de 0,20 x 13 mm. Después de 4 semanas de tratamiento, 1 sesión por semana, los resultados mostraron que un paciente tenía TMD mixto y los otros 2 miógenos; que la intensidad del dolor y la puntuación del cuestionario HADS para la ansiedad y la depresión disminuyeron en todos los pacientes y promovieron un aumento de la apertura de la boca en el paciente con limitación. En conclusión, la punción seca en los puntos gatillo activos de los músculos masetero y temporal en pacientes con TTM miofascial mostró una importante mejoría clínica después de 4 semanas de tratamiento con resolución del dolor y disminución de la ansiedad y la depresión.O presente estudo teve como objetivo avaliar a eficácia do agulhamento seco profundo para o tratamento de disfunção temporomandibular (DTM) por meio da escala numérica de dor, escala hospitalar de ansiedade e depressão e abertura bucal. Três pacientes foram avaliados e diagnosticados com DTM por exame clínico baseado Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Também foi avaliada a intensidade da dor por meio da escala numérica de dor, mensuração da abertura bucal (com paquímetro) e questionário Hospital Anxiety and Depression Scale (HADS) para avaliação de ansiedade e depressão. O agulhamento seco nos pontos gatilhos nos músculos masseter e temporal foi realizado por meio de agulhas 0,20 x 13 mm. Após 4 semanas de tratamento, 1 sessão por semana, os resultados demonstraram que uma paciente tinha DTM mista e os outros 2 miógena; que a intensidade de dor e pontuação do questionário HADS para ansiedade e depressão diminuíram para todos os pacientes e promoveu o aumento da abertura bucal na paciente que estava com limitação.
