Portrait of the use of carbapenemics in a hospital belonging to the unified health system

Resumo: | Antimicrobials are an important therapeutic arsenal against graves, as they have broad-spectrum bactericidal activities and better stability against βlactamases. This group of antimicrobials acts against gram-positive and gram-negative, aerobic and aerobic bacteria, and mainly in nosocomial infections. However, the presence of carbapenemases constitutes one of the main mechanisms of resistance. This article aims to identify the profile of patients who used carbapenem antimicrobials and quantify the consumption of this class based on the Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) methodology in a high-complexity hospital in the year 2020. This is an observational and retrospective study in a hospital belonging to the Unified Health System (SUS). Data collection was performed based on medical records, prescriptions, and antimicrobial control forms. A total of 120 patients were included in the study, the mean age was 52 ± 17.58 years, and the mean length of stay in the hospital was 33 ± 29.68 days. Carbapenem requests were analyzed, of which 74% were meropenem, 21% imipenem + cilastatin, and 5% ertapenem. The most consumed carbapenem during the study period was meropenem (DDD 252.87). Our findings made it possible to trace the profile of use of drugs belonging to the class of carbapenems in the high complexity hospital, demonstrating their importance, especially in the treatment of critically ill patients. |

Este grupo de antimicrobianos actúa contra bacterias grampositivas y gramnegativas, aerobias y anaerobias y principalmente en infecciones nosocomiales. Sin embargo, la presencia de carbapanemasas constituye uno de los principales mecanismos de resistencia. El objetivo de este trabajo es identificar el perfil de pacientes que usaron antimicrobianos carbapenémicos y cuantificar el consumo de esta clase con base en la metodología Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) en un hospital de alta complejidad en el año 2020. Se trata de un estudio observacional y retrospectivo en un hospital perteneciente al Sistema Único de Salud (SUS). La recolección de datos se realizó con base en historias clínicas, recetas médicas y formularios de control de antimicrobianos. Se incluyeron en el estudio un total de 120 pacientes, la edad media fue de 52 ± 17,58 años y la estancia media hospitalaria fue de 33 ± 29,68 días. Se analizaron las solicitudes de carbapenem, de las cuales el 74% fueron meropenem, el 21% imipenem + cilastatina y el 5% ertapenem. El carbapenem más consumido durante el período de estudio fue el meropenem (DDD 252,87). Nuestros hallazgos permitieron trazar el perfil de uso de medicamentos pertenecientes a la clase de los carbapenémicos en el hospital de alta complejidad, demostrando su importancia, especialmente en el tratamiento de pacientes críticos.Os antimicrobianos carbapenêmicos constituem um importante arsenal terapêutico contra infecções graves, pois apresentam atividades bactericidas de amplo espectro e melhor estabilidade contra β-lactamases. O objetivo desse trabalho é identificar o perfil dos pacientes que utilizaram os antimicrobianos carbapenêmicos e quantificar o consumo dessa classe tendo como base a metodologia da Anatomical Therapeutic Chemical/Dose Diária Definida (ATC/DDD) em um hospital de alta complexidade no ano de 2020. Trata-se de um estudo observacional e retrospectivo, em um hospital pertencente ao Sistema Único de Saúde (SUS). A coleta de dados foi realizada com base nos prontuários, prescrições médicas e nos formulários de controle de antimicrobianos. Ao total 120 pacientes foram incluídos no estudo, a média de idade foi de 52 ± 17,58 anos e o tempo médio de permanência na unidade hospitalar foi de 33 ± 29,68 dias. Foram analisadas as solicitações de carbapenêmicos, destes 74% eram meropenem, 21% imipenem + cilastatina e 5% ertapenem. O carbapenêmico mais consumido no período do estudo foi o meropenem (DDD 252,87).
