Skin lesions as clinical evidence for nursing care in nephrology

Resumo: Objectives: to describe the greatest clinical evidence of cutaneous manifestations of patients with chronic kidney disease by nursing professionals in nephrology; discuss how the demands arising from these injuries guide basic nursing care. Method: Convergent-care research, with twenty-one nursing professionals, in a large hospital, located in the southeastern region of Brazil, from August 2012 to August 2014. Results: Dialogue with the team in the care setting pointed out that immunosuppression and the accumulation of slag in the patient's blood generate a lot of dryness, color change, capillary fragility, itching, changes in body mass and changes related to fistula in the skin. Conclusion: Developing research combined with practice has, collectively, sensitized professionals to look / see the complex dimension of the skin as a protective envelope and unique sensory response, whose injuries demand to detail the clinical evaluation, singularize the care plan and assistance provided to the patient.

Método: Investigación de atención convergente, con veintiún profesionales de enfermería, en un gran hospital, ubicado en la región sureste de Brasil, desde agosto de 2012 hasta agosto de 2014. Resultados: Diálogo con el equipo en el entorno de atención Señaló que la inmunosupresión y la acumulación de escoria en la sangre del paciente generan mucha sequedad en la piel, cambios de color, fragilidad capilar, picazón, cambios en la masa corporal y cambios relacionados con la fístula. Conclusión: El desarrollo de la investigación combinada con la práctica, colectivamente, ha sensibilizado a los profesionales para que vean / vean la compleja dimensión de la piel como una envoltura protectora y una respuesta sensorial única, cuyas lesiones exigen detallar mejor la evaluación clínica, singularizar el plan de atención y asistencia brindada al paciente.Objetivos: descrever a maior evidência clínica das manifestações cutâneas de pacientes com doença renal crônica por profissionais de enfermagem na nefrologia; discutir como as demandas advindas dessas lesões norteiam o cuidado de enfermagem fundamental. Método: Pesquisa convergente-assistencial, com vinte e um profissionais de enfermagem, em Hospital de grande porte, localizado na região sudeste do Brasil, nos meses de agosto de 2012 a agosto de 2014. Resultados: O diálogo com a equipe no cenário do cuidado apontou que a imunossupressão e o acúmulo de escórias no sangue do paciente geram na pele muito ressecamento, alteração de coloração, fragilidade capilar, prurido, alterações de massa corporal e alterações relacionadas à fístula.
