Chronic osteomyelitis associated with mandibular fracture in a pediatric patient - case report

Resumo: Osteomyelitis is an inflammation characterized by involving the medullary spaces of the bones. In the jaw, it can occur as a result of local infections, trauma, surgical procedures and caries, affecting the mandible more than the maxilla. Patients usually present with painful symptoms, edema, trismus, fever and bone sequestration. The aim of this paper is to report a case of chronic osteomyelitis associated with mandibular fracture in a pediatric patient, going through the stages of diagnosis, drug treatment and surgery. A five-year-old female patient was referred to the hospital unit with a history of inflammation in the left mandibular region and a fall from her own height. On physical examination, he presented edema in the left hemiface and fistula in the left mandibular angle region with active drainage of a purulent collection. On intraoral examination, he presented dental elements with extensive destruction by caries. The tomographic examination showed a process of osteolysis, spinal cord destruction and mandibular fracture, which, associated with the patient's history and clinical characteristics, led to the diagnostic hypothesis of mandibular osteomyelitis associated with pathological fracture. Initially, antimicrobial therapy was instituted aiming at a less aggressive and more conservative surgical intervention, and we opted for conservative treatment of the fracture in the mandible angle region. The surgical procedure consisted of debridement of necrotic tissue and extractions. It is known that osteomyelitis in pediatric patients is relatively uncommon and that it can generate conditions that are difficult to resolve, especially when not treated quickly and early. Diagnostic delays can evolve and reach prime anatomical areas such as the cervical region and mediastinum, leading to the risk of death.

En la mandíbula, puede ocurrir como resultado de infecciones locales, traumatismos, procedimientos quirúrgicos Y caries, afectando más a la mandíbula que al maxilar. Los pacientes suelen presentar síntomas dolorosos, edema, trismo, fiebre Y secuestro óseo. El objetivo de este trabajo es reportar un caso de osteomielitis crónica asociada a fractura mandibular en un paciente pediátrico, pasando por las etapas de diagnóstico, tratamiento farmacológico Y cirugía. Paciente de sexo femenino de cinco años que fue remitida a la unidad hospitalaria con antecedente de inflamación en la región mandibular izquierda Y caída desde su propia altura. Al examen físico presentaba edema en la hemifacial izquierda Y fístula en la región del ángulo mandibular izquierdo con drenaje activo de una colección purulenta. Al examen intraoral presentó elementos dentarios con gran destrucción por caries. El examen tomográfico mostró un proceso de osteólisis, destrucción medular Y fractura mandibular, que asociado a la historia Y características clínicas de la paciente, llevó a la hipótesis diagnóstica de osteomielitis mandibular asociada a fractura patológica. Inicialmente, se instituyó la terapia antimicrobiana con el objetivo de una intervención quirúrgica menos agresiva Y más conservadora, Y se optó por un tratamiento conservador de la fractura en la región del ángulo mandibular. El procedimiento quirúrgico consistió en desbridamiento del tejido necrótico Y extracciones. Se sabe que la osteomielitis en pacientes pediátricos es relativamente infrecuente Y que puede generar condiciones de difícil resolución, especialmente cuando no se tratan de forma rápida Y precoz. Los retrasos en el diagnóstico pueden evolucionar y alcanzar áreas anatómicas primordiales como la región cervical y el mediastino, lo que conlleva riesgo de muerte.A osteomielite é uma inflamação caracterizada por envolver os espaços medulares dos ossos. Nos maxilares, pode ocorrer como resultado de infecções locais, traumas, procedimentos cirúrgicos e cáries, acometendo mais a mandíbula do que a maxila. Os pacientes costumam apresentar sintomatologia dolorosa, edema, trismo, febre e sequestros ósseos. O objetivo deste trabalho é relatar um caso de osteomielite crônica associada à fratura mandibular em paciente pediátrico, passando pelas etapas de diagnóstico, tratamento medicamentoso e cirúrgico. Paciente de cinco anos, do sexo feminino, foi encaminhada à unidade hospitalar com histórico de quadro inflamatório em região mandibular esquerda e queda da própria altura. Ao exame físico, apresentava edema em hemiface esquerda e fístula em região de ângulo mandibular esquerdo com drenagem ativa de coleção purulenta. Ao exame intraoral, apresentava elementos dentários com ampla destruição por cárie. O exame tomográfico evidenciou processo de osteólise, destruição medular e fratura mandibular que, associados ao histórico da paciente e suas características clínicas, chegou-se à hipótese diagnóstica de osteomielite mandibular associada à fratura patológica. Inicialmente, foi instituída terapia antimicrobiana visando uma intervenção cirúrgica menos agressiva e mais conservadora, e optou-se por tratamento conservador da fratura em região de ângulo de mandíbula. O procedimento cirúrgico consistiu em desbridamento de tecido necrótico e exodontias. Sabe-se que a osteomielite em pacientes pediátricos é relativamente incomum e que pode gerar quadros de difícil resolução, sobretudo quando não tratados rápida e precocemente.
