Use of metformin as monotherapy in the management of gestational Diabetes mellitus

Resumo: Gestational diabetes mellitus (GDM) is a disorder characterized by a state of glucose intolerance, detected for the first time during pregnancy, resulting in hyperglycemia of variable levels, which may or may not persist after birth. For most women with GDM, monitoring, nutritional guidance and lifestyle changes are sufficient for treatment. However, some women may require drug treatment. Oral antidiabetics (OAD), such as metformin, are not first-choice drugs for the treatment of GDM, but they can be considered in some situations. This work aims to obtain the therapeutic success rate of monotherapy with metformin in pregnant women with GDM, in addition to analyzing the presence of obesity and advanced maternal age as possible factors associated with the disease and treatment. This is a descriptive research, with a quantitative approach, through analysis of medical records of pregnant women with GDM. The results of this study demonstrated that metformin monotherapy was not effective in more than half of the cases, and highlighted the importance of following the most current recommendations for the treatment of GDM. The vast majority of pregnant women (91.8%) had a gestational BMI that constitutes overweight or obesity, highlighting the presence of excess weight as a determining factor in the development of the disease, and the need for adequate prenatal care.

Para la mayoría de las mujeres con DMG, el seguimiento, la orientación nutricional y los cambios en el estilo de vida son suficientes para el tratamiento. Sin embargo, algunas mujeres pueden necesitar tratamiento farmacológico. Los antidiabéticos orales (ADO), como la metformina, no son fármacos de primera elección para el tratamiento de la DMG, pero pueden considerarse en algunas situaciones. Este trabajo tiene como objetivo obtener la tasa de éxito terapéutico de la monoterapia con metformina en mujeres embarazadas con DMG, además de analizar la presencia de obesidad y edad materna avanzada como posibles factores asociados a la enfermedad y al tratamiento. Se trata de una investigación descriptiva, con enfoque cuantitativo, mediante el análisis de historias clínicas de gestantes con DMG. Los resultados de este estudio demostraron que la monoterapia con metformina no fue efectiva en más de la mitad de los casos y resaltaron la importancia de seguir las recomendaciones más actuales para el tratamiento de la DMG. La gran mayoría de las gestantes (91,8%) presentó un IMC gestacional que constituye sobrepeso u obesidad, destacándose la presencia de exceso de peso como factor determinante en el desarrollo de la enfermedad, y la necesidad de una adecuada atención prenatal.O diabetes mellitus gestacional (DMG) é um distúrbio caracterizado por um estado de intolerância à glicose, detectada pela primeira vez na gestação, resultando em hiperglicemia de níveis variáveis, podendo ou não persistir após o parto. Para a maioria das mulheres com DMG, o monitoramento, orientação nutricional e mudança no estilo de vida são suficientes para o tratamento. Entretanto, algumas mulheres podem necessitar de tratamento medicamentoso. Os antidiabéticos orais (ADO), como a metformina, não são drogas de primeira escolha para o tratamento de DMG, mas podem ser considerados em algumas situações. Este trabalho tem como objetivo obter a taxa de sucesso terapêutico da monoterapia com metformina em gestantes com DMG, além de analisar a presença de obesidade e idade materna avançada como possíveis fatores associados à doença e ao tratamento. Trata-se de uma pesquisa descritiva, com abordagem quantitativa, mediante análise de prontuários de gestantes com DMG. Os resultados deste estudo demonstraram que a monoterapia com metformina não foi eficaz em mais da metade dos casos, e destacaram a importância do seguimento das recomendações mais atuais para o tratamento do DMG.
