Remote monitoring of patients with chronic heart failure in times of social distance - COVID-19 in the light of the “Primary Nursing” model

Resumo: This article aims to describe the contingency plan of a specialized clinic for patients with chronic heart failure to act during the COVID-19 pandemic. This is an experience report on the actions developed through remote care of heart failure patients and their caregivers, based on the Primary Nursing model, in a clinic located in Niterói/RJ/Brazil. The intervention has been implemented since March 2020 in a web system called “Sistema Coração Valente” and the data is being stored in a database that uses MySQL. Obtained as a result the report of the actions used during the isolation of social faith described chronologically, considering the planning, implementation and evaluation of results. Then, it is concluded the contingency plan of the “Clínica de Insuficiência Cardíaca Coração Valente” for telephone consultations, based on the Primary Nursing model, proved to be effective in the remote monitoring of patients with heart failure.

Este es un informe de experiencia sobre las acciones desarrolladas a través de la atención remota para pacientes con insuficiencia cardíaca crónica y sus cuidadores, basado en el modelo Primary Nursing, en una clínica ubicada en Niterói/RJ/Brasil. La intervención se implementó desde marzo de 2020 en un sistema web llamado "Sistema Coração Valente" y los datos se almacenan en una base de datos que utiliza MySQL. Obtenido como resultado el informe de las acciones utilizadas durante el aislamiento social se describió cronológicamente, considerando la planificación, implementación y evaluación de resultados. Luego concluye el plan de contingencia para consultas telefónicas de la Clínica de Insuficiencia “Coração Valente”, basado en el modelo Primary Nursing, demostró ser eficaz en la monitorización remota de pacientes con insuficiencia cardíaca.Este artigo tem por objetivo descrever o plano de contingência de uma clínica especializada em atendimento de pacientes com insuficiência cardíaca crônica para atuação durante a pandemia por COVID-19. Trata-se de um relato de experiência sobre as ações desenvolvidas através de atendimento remoto à pacientes com insuficiência cardíaca crônica e seus cuidadores, fundamentadas no modelo Primary Nursing, em uma clínica localizada em Niterói/RJ/Brasil. A intervenção tem sido implementada desde março de 2020 em um sistema web denominado “Sistema Coração Valente” e os dados estão sendo armazenados em um banco de dados que utiliza o MySQL. Obteve-se como resultado o relato das ações utilizadas durante o isolamento social sendo descrito cronologicamente, considerando o planejamento, a implementação e a avaliação de resultados.
