Evolution analysis of segmental body composition in bariatric patients with weight regain and undergoing pharmacological therapy with liraglutide

Resumo: | Obesity is a chronic, multifactorial disease, responsible for several morbidities, and bariatric surgery is an efficient alternative for rapid weight loss. About 20 to 25% of patients, even losing weight after the procedure, tend to regain weight after the second year. This study aimed to analyze the evolution of segmental body composition in bariatric patients with weight regain and undergoing pharmacological therapy with liraglutide. This is a retrospective, longitudinal study through data analysis in the medical records of an institution specialized in clinical and surgical treatment of obesity. The sample was comprised of 22 bariatric patients treated between January 2016 and December 2019. Anthropometric data from segmental impedance measurements were collected, as well as data on the clinical evolution regarding the proposed treatment. The results showed that in both doses, the loss of mass was relevant in all segments, indicating significance. BMI reduced 9.3% in all patients in the 24 weeks analyzed. With 2.4mg/day of liraglutide, there was a 7.2% weight loss and, with a 3.0mg/day dose, the loss was 9.84% of the weight. Considering both doses, the overall weight loss was 9.2%.The percentages of mean differences in fat were similar between trunk and limbs. With respect to lean mass, although there was no statistical significance, we have a clinical significance of protection. |

Alrededor del 20 al 25% de los pacientes, incluso perdiendo peso después del procedimiento, tienden a recuperar peso después del segundo año. Este estudio tuvo como objetivo analizar la evolución de la composición corporal segmentaria en pacientes bariátricos con recuperación de peso y en tratamiento farmacológico con liraglutida. Se trata de un estudio longitudinal retrospectivo a través del análisis de datos en la história clínica de una institución especializada en el tratamiento clínico y quirúrgico de la obesidad. La muestra estuvo conformada por 22 pacientes bariátricos tratados entre enero de 2016 y diciembre de 2019. Se recolectaron datos antropométricos de las medidas de impedancia segmentaria, así como datos sobre la evolución clínica respecto al tratamiento propuesto. Los resultados mostraron que en ambas dosis, la pérdida de masa fue relevante en todos los segmentos, indicando significancia. El IMC se redujo un 9,3% en todos los pacientes en las 24 semanas analizadas. Con 2,4 mg / día de liraglutida, hubo una pérdida de peso del 7,2% y, con una dosis de 3,0 mg / día, la pérdida fue del 9,84% del peso. Considerando ambas dosis, la pérdida de peso global fue del 9,2%. Los porcentajes de diferencias medias en la grasa fueron similares entre el tronco y las extremidades. Con respecto a la masa magra, aunque no hubo significación estadística, tenemos una significación clínica de protección.A obesidade é uma doença crônica, multifatorial, responsável por diversas morbidades, sendo a cirurgia bariátrica uma alternativa eficiente para a perda de peso. Cerca de 20% a 25% dos pacientes, mesmo perdendo peso após o procedimento, tendem a apresentar reganho de peso após o segundo ano. Este estudo teve como objetivo central analisar a evolução da composição corporal segmentar em bariátricos com reganho de peso e submetidos à terapêutica farmacológica com liraglutida. Trata-se de um estudo retrospectivo, longitudinal, por meio de análise de dados em prontuário de uma instituição especializada em terapêutica clínica e cirúrgica da obesidade. A amostra foi composta por 22 pacientes bariátricos atendidos entre janeiro de 2016 a dezembro de 2019. Foram coletados dados antropométricos de impedanciometria segmentar, bem como dados da evolução clínica referente ao tratamento proposto. Em ambas as doses, a perda de massa se mostrou relevante em todos os segmentos, apontando significância. O IMC reduziu 9,3% em todos os pacientes nas 24 semanas analisadas. Com 2,4mg/dia de liraglutida, houve perda de 7,2% do peso e, com dose de 3,0mg/dia, esta perda foi de 9,84%. Considerando ambas as doses, a perda de peso geral foi de 9,2%. As porcentagens de diferenças médias de gordura foram similares entre o tronco e os membros.
