Clinical aspects and main procedures performed in hypertensive urgencies and emergencies: a narrative review

Resumo: Hypertensive crisis is a significant clinical entity with considerable mortality and morbidity, with organ damage in many patients, a factor that significantly hinders quality of life. In view of this, the importance of urgent and emergency care plan in situations of hypertensive crisis is highlighted, in order to ensure immediate care that can offer a rapid and gradual reduction in blood pressure levels. The objective of the study was to offer a brief narrative review on the approach of the hypertensive crisis in the face of the terminologies 'hypertensive urgency' and 'hypertensive emergency', in order to understand the clinical aspects and the main conducts performed in the care, articulating with the performance of the professional nurse facing this clinical condition. This research was elaborated from a bibliographic survey in the scientific databases of the Scientific Electronic Library Online (SCIELO), Latin American and Caribbean Literature on Health Sciences (LILACS), Google Scholar and Virtual Health Library (VHL), with publications between 2015 and 2020 as a temporal delimitation. In view of the results obtained, it was found that all articles presented an approach to hypertensive crisis in urgency and emergency services. Noting that both hypertensive crisis terminologies are used, as well as the terms 'hypertensive urgency' and 'hypertensive emergency,' to present the peculiarities and relevant results about each condition. It is also important to highlight the need for better care by the nursing team, mainly because they deal directly and frequently with patients, using risk classification as an essential tool in redirecting the flow of care in urgent and emergency services. The demands of hypertensive crisis in the urgency and emergency sector signal the need to improve local health promotion and prevention actions, with primary health care playing a fundamental role in the development of these strategies.

Frente a eso, se destaca la importancia de plan de atención de urgencia y emergencia en situaciones de crisis hipertensiva, con el fin de garantizar cuidados inmediatos que puedan ofrecer una reducción rápida y gradual de los niveles de presión arterial. El objetivo del estudio fue ofrecer una breve revisión narrativa sobre el abordaje de la crisis hipertensiva frente a las terminologías 'urgencia hipertensiva' y 'emergencia hipertensiva', con el fin de comprender los aspectos clínicos y las principales conductas realizadas en la atención, articulando con la actuación del profesional de enfermería frente a esta condición clínica. Esta investigación se elaboró a partir de un levantamiento bibliográfico en las bases de datos científicas de la Biblioteca Científica Electrónica en Línea (SCIELO), Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS), Google Scholar y Biblioteca Virtual en Salud (BVS), con publicaciones entre 2015 y 2020. A la vista de los resultados obtenidos, se constató que todos los artículos presentaron un abordaje de la crisis hipertensiva en los servicios de urgencia y emergencia. Obsérvese que se utilizan ambas terminologías de crisis hipertensiva, así como los términos 'urgencia hipertensiva' y 'emergencia hipertensiva', para presentar las peculiaridades y resultados relevantes sobre cada condición. También es importante resaltar la necesidad de una mejor atención por parte del equipo de enfermería, principalmente porque atienden de manera directa y frecuente a los pacientes, utilizando la clasificación de riesgo como herramienta fundamental para redirigir el flujo de atención en los servicios de urgencia y emergencia. Las exigencias de la crisis hipertensiva en el sector de urgencias y emergencias señalan la necesidad de mejorar las acciones locales de promoción y prevención de la salud, teniendo la atención primaria de salud un papel fundamental en el desarrollo de estas estrategias.A crise hipertensiva é uma entidade clínica significativa com considerável mortalidade e morbidade, apresentando lesões de órgãos em muitos pacientes, fator este que dificulta significativamente a qualidade de vida. O objetivo foi oferecer uma breve revisão narrativa sobre a abordagem da crise hipertensiva diante das terminologias ‘urgência hipertensiva’ e ‘emergência hipertensiva’, a fim de compreender sobre os aspectos clínicos e as principais condutas realizadas no atendimento, articulando com a atuação do profissional enfermeiro frente a esta condição clínica. Esta pesquisa foi elaborada a partir de levantamento bibliográfico nas bases de dados científicas das bibliotecas virtuais Scientific Electronic Library Online (SCIELO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Google Acadêmico e Biblioteca Virtual de Saúde (BVS), tendo como delimitação temporal as publicações entre 2015 e 2020. Diante dos resultados obtidos, verificou-se que todos os artigos apresentaram uma abordagem sobre crise hipertensiva nos serviços de urgência e emergência. Verificando-se que tanto são utilizadas terminologias de crise hipertensiva, como também são utilizados os termos ‘urgência hipertensiva’ e ‘emergência hipertensiva’. É relevante destacar sobre a necessidade de um melhor atendimento por parte da equipe de enfermagem, porque lidam diretamente e frequentemente com os pacientes, utilizando a classificação de risco como instrumento essencial no redirecionamento do fluxo do atendimento em serviços de urgência e emergência.
